Summary

Her demonstrerer vi akupunktanvendelse kombineret med akupunktmassage til behandling af forstoppelse hos en patient med kronisk obstruktiv lungesygdom (KOL).

Abstract

Kronisk obstruktiv lungesygdom (KOL) er en almindelig og hyppig sygdom hos ældre patienter med tendens til progressiv forværring. Forstoppelse, der nedsætter livskvaliteten og udløser risikoen for sygdomme, er et almindeligt samtidigt symptom hos patienter med KOL. I øjeblikket opnår vestlig medicinsk behandling ikke de ønskede resultater for patienterne. En høj tilbagefaldsrate ledsager det, mens traditionel kinesisk medicin (TCM) har en lang historie og rig erfaring med behandling af kroniske sygdomme. Både akupunktapplikation og akupunktmassage er karakteristiske terapier af TCM, med mindre bivirkninger, høj sikkerhed, enkel betjening og fremragende fordele. De er effektive til behandling af forstoppelse for patienter med KOL og betragtes som en ideel alternativ behandling for patienter med kronisk forstoppelse. Formålet med denne artikel er at introducere metoden til akupunktapplikation kombineret med akupunktmassage til behandling af forstoppelse hos patienter med KOL, herunder valg af punkter, emner, behandlingstid og operationsprocedure.

Introduction

Kroniske luftvejssygdomme er den tredjehyppigste dødsårsag i verden, og KOL er den hyppigste dødsårsag som følge af kroniske luftvejssygdomme, hvilket lægger en tung byrde på både den enkelte og samfundet1. Kina har det største antal patienter med KOL i verden: ca. 99 millioner, med en prævalens på 8,6%2. Forstoppelse er en almindelig komplikation hos KOL-patienter og er forbundet med høje niveauer af polyfarmaci3. Der er tegn på, at mere end 39% af KOL-patienter oplever forstoppelse, og forstoppelse var positivt korreleret med sværhedsgraden af dyspnø, tidspunktet for eksacerbationer og komplikationer af KOL4. Det er stærkt mistanke om, at forstoppelse er forbundet med akutte forværringer af kronisk obstruktiv lungesygdom (AECOPD). AEKOL accelererer sygdomsprogressionen hos KOL-patienter og er den hyppigste dødsårsag hos KOL-patienter5. Derfor er det på tide at være opmærksom på behandlingen af forstoppelse hos patienter med KOL.

De fleste patienter håndterer deres forstoppelse alene, hovedsageligt ved hjælp af kostjusteringer og afføringsmidler. Undersøgelser har vist, at osmotiske afføringsmidler ofte er det valgte lægemiddel for mennesker med forstoppelse6. Imidlertid er brugen af medicin undertiden ikke effektiv til forbedring af forstoppelsessymptomer. Det er forbundet med bivirkninger eller dårlig tolerance. Op til 50% af patienter med forstoppelse er ikke helt tilfredse med resultaterne af deres nuværende behandlinger 7,8. Derfor er det nødvendigt at finde alternative behandlingsformer med god effekt og høj sikkerhed. Ved behandling af kronisk forstoppelse begynder akupunktstimuleringsterapi at vise nogle fordele9. Med vejledning af begrebet holisme, syndromdifferentiering og behandling samt meridianakupunktteori i TCM bruges både akupunktapplikation og akupunktmassage til behandling af sygdomme ved at stimulere specifikke akupunkturpunkter på kropsoverfladen10,11. Acupoint ansøgning kan gøre stoffet direkte absorberet af den lokale hud. Det opnår effekten af kanalisering af meridianer og harmonisering af qi og blod ved at stimulere akupunkturpunkterne og derved forbedre symptomerne på forstoppelse10. Acupoint massage stimulerer den sakrale nerve, regulerer afføring ved at øge det interstitielle celleantal og kontrollere rytmiske sammentrækninger eller peristaltik gennem aktivering af afføring12. Talrige farmakologiske og kliniske undersøgelser har bekræftet den klare virkning af urter som Rheum officinale Baill til behandling af forstoppelse13,14. En demonstration af akupunktapplikation kombineret med akupunktmassage til behandling af forstoppelse hos en patient med kronisk obstruktiv lungesygdom er præsenteret i denne artikel.

Protocol

Den kliniske undersøgelsesprotokol blev gennemgået og godkendt af Clinical Research Approval Committee ved Department of Respiratory Medicine, Hospital of Chengdu University of Traditional Chinese Medicine (Record No. KY2022014). 1. Patienter med KOL diagnosticeres og behandles i henhold til “The Global Initiative for Chronic Obstructive Lung Disease”, udgivet i 202316. De diagnostiske kriterier for forstoppelse henviste til “Retningslinje for klinisk praksis om evaluering og styring af kronisk forstoppelse for kinesiske voksne” udstedt af Chinese Society of Colorectal Surgery, Chinese Society of Surgery, Chinese Medical Association i 202217. Discussion

Den ældre befolkning er i høj risiko for KOL. Ældre mennesker oplever varierende grader af gastrointestinal tilbagegang, mens hypoxi og nedsat aktivitet hos KOL-patienter øger risikoen for forstoppelse.
