La prueba de activación basófila es una prueba diagnóstica in vitro complementaria para la evaluación de reacciones alérgicas mediadas por IgE basada en la detección de la activación basófila en presencia de un estímulo específico mediante la medida de marcadores de activación mediante citometría de flujo.
La prueba de activación de basófilos (BAT) es una prueba diagnóstica in vitro complementaria que se puede utilizar además de la historia clínica, la prueba cutánea (ST) y la determinación específica de IgE (sIgE) en la evaluación de reacciones alérgicas mediadas por IgE a alimentos, veneno de insectos, medicamentos, así como algunas formas de urticaria crónica. Sin embargo, el papel de esta técnica en los algoritmos de diagnóstico es muy variable y no está bien determinado.
Las MTD se basan en la determinación de la respuesta basófila a la activación de IgE de reticulación alérgeno/fármaco mediante la medición de marcadores de activación (como CD63, CD203c) mediante citometría de flujo. Esta prueba puede ser una herramienta útil y complementaria para evitar las pruebas de provocación controladas para confirmar el diagnóstico de alergia, especialmente en sujetos que experimentan reacciones graves potencialmente mortales. En general, se debe considerar el rendimiento de las MTD si: i) el alérgeno/fármaco produce resultados falsos positivos en el ST; ii) no hay ninguna fuente de alérgenos/medicamentos para la determinación de ST o sIgE; iii) existe discordancia entre la historia del paciente y la determinación del ST o la sIgE; iv) los síntomas sugieren que el ST puede dar lugar a una respuesta sistémica; v) antes de considerar un CCT para confirmar el alérgeno / medicamento culpable. Las principales limitaciones de la prueba están relacionadas con la sensibilidad no óptima, especialmente en la alergia a medicamentos, la necesidad de realizar la prueba no más de 24 h después de la extracción de la muestra y la falta de estandarización entre laboratorios en términos de procedimientos, concentraciones y marcadores celulares.
El diagnóstico de alergia mediada por IgE se basa en la historia clínica, las pruebas cutáneas (ST), la cuantificación de la IgE específica sérica (sIgE) y, si es necesario e indicado, las pruebas de provocación controlada (ECC)1,2,3,4,5,6. Sin embargo, la historia clínica puede ser poco confiable ya que puede haber una falta de información precisa, y los ST y CCT no son procedimientos libres de riesgo que pueden estar contraindicados en sujetos que experimentan reacciones graves potencialmente mortales 1,2,3,4,5,6 . Estas cuestiones, junto con el hecho de que la determinación de sIgE mediante inmunoensayos de fluoroenzimas validados y comerciales solo está disponible para unos pocos alérgenos y fármacos, han puesto de relieve el importante papel de otros ensayos funcionales in vitro, como la prueba de activación basófila (MTD).
Los basófilos son células efectoras clave involucradas en reacciones alérgicas mediadas por IgE que se activan tras la reticulación de sIgE adyacente unida a receptores de alta afinidad (FcεRI) en la superficie celular después de la exposición a alérgenos / medicamentos. La activación basófila desencadena la desgranulación celular y la liberación de mediadores inflamatorios preformados y nuevos sintetizados contenidos en gránulos de secreción intracitoplasmática 7,8,9. BAT es un método in vitro que intenta imitar la activación basófila en presencia de un estímulo (alérgeno o fármaco) y determina cambios en la expresión de marcadores de activación basófilos mediante citometría de flujo 7,10. Existen diferentes estrategias para identificar basófilos (IgE+, CCR3+, CRTH2+, CD203c+) y medir la activación celular (principalmente regulación positiva de CD63 y CD203c) utilizando combinaciones de anticuerpos marcados con fluorocromo 7,10. CD63, el mejor marcador de activación clínicamente validado 11,12,13,14, es una proteína de membrana anclada en los gránulos secretores que contienen histamina que, después de la activación celular y la fusión de los gránulos con la membrana, se expresa en la superficie basófila 15,16,17,18,19,20,21 . CD203c es un marcador de superficie que se expresa constitutivamente en basófilos y se regula al alza después de la estimulación de FcεRI, que también ha mostrado resultados confiables en BAT 15,22,23,24,25. Además, parece co-expresarse con CD6326.
En las últimas décadas, las MTD han demostrado ser útiles en el diagnóstico de reacciones alérgicas mediadas por IgE inducidas por diferentes desencadenantes como medicamentos, alimentos o inhalantes, así como en algunas formas de urticaria crónica, como se describe a continuación. Sin embargo, la posición de esta técnica en los algoritmos diagnósticos es muy variable y no está bien determinada.
Hipersensibilidad a los medicamentos
Las MTD han demostrado ser útiles como prueba complementaria para fármacos y pacientes seleccionados, especialmente para aquellos que experimentan reacciones graves debido al hecho de que el valor diagnóstico del ST no está bien establecido para la mayoría de los medicamentos, ya que están validados y estandarizados para un número limitado de fármacos27,28,29,30. Además, la cuantificación de sIgE está disponible sólo para un número limitado de fármacos, con menor sensibilidad que ST 27,28,29,30,31,32. Por lo tanto, el diagnóstico de hipersensibilidad a los medicamentos generalmente se basa en la prueba de provocación de drogas, que puede estar contraindicada en sujetos que experimentan reacciones graves que amenazan la vida33.
Se han reportado resultados prometedores para el uso de MTD en pacientes seleccionados que informaron reacciones de hipersensibilidad inmediata a diferentes fármacos como betalactámicos (BL)20,34,35,36,37,38,39, agentes bloqueadores neuromusculares (NMBA)19,22,40,41,42, 43,44,45, fluoroquinolonas (FQ)46,47,48,49, pirazolonas 50,51,52, medios de radiocontraste (RCM)53,54,55,56 y compuestos de platino57,58,59 . Se ha informado que las MTD tienen una sensibilidad y especificidad entre 51,7-66,9% y 89,2-97,8%, respectivamente; y se describen valores predictivos positivos y negativos que oscilan entre 93,4% y 66,3%, respectivamente27,31. Además, el BAT ha sido propuesto como un biomarcador predictivo para reacciones innovadoras durante la desensibilización con compuestos de platino, ya que la expresión de CD203c está aumentada en comparación con CD63 en pacientes con alto riesgo de reacciones adversas durante la desensibilización del fármaco57.
Cabe destacar que las MTD sólo son útiles en la hipersensibilidad a los medicamentos cuando la reacción implica la degranulación basófila; Por lo tanto, no es útil en reacciones resultantes de la inhibición enzimática de la ciclooxigenasa 142.
Alergia alimentaria
Las MTD se han convertido en una herramienta diagnóstica potencial para la alergia alimentaria porque la determinación de la sIgE sérica para todo el extracto alergénico o alérgenos individuales es a menudo ambigua, lo que requiere un desafío alimentario oral para confirmar el diagnóstico, que, de manera similar a la hipersensibilidad al medicamento, es un procedimiento costoso y no exento de riesgos60. Varios estudios han mostrado resultados relevantes con leche de vaca 61,62, huevo 61,63, trigo 64,65,66,67,68, cacahuete 63,69,70,71,72, avellana 73,74,75,76 ,77, mariscos78, melocotón 79,80,81, manzana 21, apio y zanahoria82,83.
El principal valor añadido de las MTD en el diagnóstico de la alergia alimentaria en comparación con las ST y la IgE en suero es que muestra una mayor especificidad y una sensibilidad similar. Por lo tanto, las MTD son una herramienta útil para diferenciar a los pacientes clínicamente alérgicos de los sujetos sensibilizados, pero tolerantes, que tienen alta especificidad (75-100%) y sensibilidad (77-98%)63,69,84. Los valores de sensibilidad y especificidad dependen del alérgeno y otros factores como fenotipos (p. ej., síndrome de alergia oral versus anafilaxia), edad y patrones de sensibilización relacionados con la geografía63,85.
Las MTD que utilizan componentes de alérgenos individuales pueden mejorar potencialmente la precisión diagnóstica de algunos alérgenos alimentarios61,80. Existen estudios que utilizan proteínas de almacenamiento de semillas (por ejemplo, Ara h 1, Ara h 2, Ara h 3 y Ara h 6 del maní)86; proteínas de transferencia de lípidos (p. ej., Pru p 3 del melocotón y Ara h 9 del maní)80,86; y homólogos de Bet v 1 (por ejemplo, Ara h 8 de maní)87. Otras utilidades potenciales están relacionadas con la identificación del alérgeno culpable en casos de síndrome de alergia polen-alimentos 21,87,88, alergia a la carne roja 89 o anafilaxia inducida por el ejercicio dependiente de alimentos66.
Curiosamente, las MTD pueden proporcionar información sobre la gravedad y el umbral de las reacciones alérgicas, ya que los pacientes con reacciones más graves muestran una mayor proporción de basófilos activados, como se observó en estudios de pacientes alérgicos al maní y a la leche de vaca 84,90,91; y los pacientes que reaccionan a trazas del alérgeno muestran una mayor sensibilidad basófila 84,90,92. Estos datos sugieren que las MTD pueden ser útiles para identificar a los pacientes alérgicos de alto riesgo que requieren un seguimiento más estrecho y una educación más intensa93. Además, se ha informado que las MTD pueden predecir las respuestas al desafío alimentario 70,91,92,94 y los umbrales de reactividad90,95 para ayudar a determinar cuándo los alimentos pueden ser (re)introducidos de manera segura 84. Sin embargo, estos hallazgos son controvertidos en algunos estudios63,96 y se requiere más investigación.
Por otro lado, las MTD se han utilizado para monitorizar la resolución de la alergia alimentaria, ya sea de forma natural o bajo tratamientos inmunomoduladores, a lo largo del tiempo, que hasta ahora solo se ha evaluado mediante provocación alimentaria oral, con los riesgos y costes asociados 84,97,98,99,100,101,102,103,104,105,106 ,107,108. Además, también se ha utilizado para controlar el efecto de omalizumab en la alergia alimentaria, ya que la activación basófila disminuye durante el tratamiento con omalizumab, pero aumenta después del cese del tratamiento109.
Alergia por inhalación
Las MTD rara vez son beneficiosas en la alergia por inhalación, ya que el diagnóstico se puede establecer de forma rutinaria mediante la cuantificación de sIgE y ST. Sin embargo, en casos de rinitis alérgica local (niveles indetectables de sIgE y ST negativos con pruebas de provocación nasal positivas), la MTD ha permitido el diagnóstico en el 50% de los casos110. Se ha reportado además una correlación entre la sensibilidad basófila y la respuesta a las pruebas de provocación nasal/bronquial, así como entre la gravedad del asma y la eficacia del tratamiento con omalizumab111,112.
Las MTD también se han utilizado para controlar la inmunoterapia con alérgenos para los ácaros del polvo doméstico y el polen, ya que la sensibilidad basófila disminuye durante la inmunoterapia, probablemente debido a la interferencia del bloqueo de anticuerpos IgG 113,114,115,116,117.
Alergia al veneno de himenópteros
El diagnóstico de la alergia al veneno de himenópteros se basa rutinariamente en ST y sIgE sérica. La MTD ha mostrado una alta sensibilidad (85-100%) y especificidad (83-100%) y se ha informado que es útil en casos que arrojan resultados equívocos o en pacientes con una historia clínica sugestiva de alergia al veneno pero sIgE indetectable y ST118,119 negativo. Sin embargo, las MTD no parecen ser predictivas de la gravedad de estas reacciones120,121.
Hasta el 60% de los pacientes presentan sIgE tanto para el veneno de avispa como de abeja, y la identificación del alérgeno dominante es crucial para un tratamiento adecuado de inmunoterapia. En estos casos, se ha informado que las MTD son útiles para identificar el alérgeno dominante119,122,123,124. Aunque la sIgE a los principales alérgenos de los venenos de abeja y avispa puede reducir la utilidad de las MTD en pacientes con doble positividad a ambos venenos, proporciona información útil principalmente en sujetos con resultados negativos en las determinaciones de IgE123.
Algunos estudios sugieren que las MTD pueden ser útiles como biomarcador predictivo de los efectos secundarios durante la fase de acumulación de la inmunoterapia con veneno, ya que se ha informado que esta opción de tratamiento disminuye la sensibilidad basófila. Sin embargo, la reactividad no disminuye y esta utilidad BAT es hoy controvertida 13,120,125,126,127,128,129,130.
Urticaria y angioedema
Un subconjunto de pacientes con urticaria crónica tiene una fisiopatología autoinmune, debido a los autoanticuerpos IgE contra autoalérgenos y autoanticuerpos IgG que se dirigen a los complejos FcεRI o IgE-FcεRI presentes en la superficie de los mastocitos131,132. En la práctica clínica, el diagnóstico de este tipo de urticaria crónica se ha basado en ST sérico autólogo positivo, que tiene riesgo de infección accidental. Las MTD se han propuesto como una prueba in vitro para diagnosticar y monitorizar a los pacientes con sospecha de urticaria crónica. Se ha informado que la expresión de CD63 y CD203c en la superficie de los basófilos aumenta después de la estimulación con sueros de pacientes con urticaria crónica, mostrando la detección de autoanticuerpos activos 133,134,135,136,137. Recientemente, se ha informado que los pacientes con BAT positivas a menudo experimentan el estado de enfermedad más activo, evaluado por la puntuación de actividad de la urticaria, y requieren dosis más altas de antihistamínicos junto con tratamientos de tercera línea (ciclosporina A u omalizumab), en comparación con aquellos con BAT138 negativo.
BAT es una prueba diagnóstica in vitro complementaria para la evaluación de reacciones alérgicas mediadas por IgE que ha demostrado ser útil en el diagnóstico de reacciones inducidas por diferentes desencadenantes como medicamentos, alimentos o inhalantes, así como en algunas formas de urticaria crónica.
