Le test d’activation des basophiles est un test de diagnostic in vitro complémentaire pour l’évaluation des réactions allergiques médiées par les IgE basé sur la détection de l’activation des basophiles en présence d’un stimulus spécifique par la mesure des marqueurs d’activation par cytométrie en flux.
Le test d’activation des basophiles (MTD) est un test diagnostique in vitro complémentaire qui peut être utilisé en plus des antécédents cliniques, du test cutané (ST) et de la détermination des IgE spécifiques (IgE) dans l’évaluation des réactions allergiques médiées par les IgE aux aliments, au venin d’insecte, aux médicaments, ainsi qu’à certaines formes d’urticaire chronique. Cependant, le rôle de cette technique dans les algorithmes de diagnostic est très variable et mal déterminé.
La MTD est basée sur la détermination de la réponse basophile à l’activation des IgE de réticulation allergène/médicament par la mesure de marqueurs d’activation (tels que CD63, CD203c) par cytométrie en flux. Ce test peut être un outil utile et complémentaire pour éviter les tests de provocation contrôlés afin de confirmer le diagnostic d’allergie, en particulier chez les sujets présentant des réactions graves mettant la vie en danger. En général, l’efficacité de la MTD doit être prise en compte si i) l’allergène/médicament produit des résultats faussement positifs dans le ST; ii) il n’y a pas de source d’allergène/médicament à utiliser pour la détermination des ST ou des IgE; iii) il existe une discordance entre les antécédents du patient et la détermination des ST ou des IgE; iv) les symptômes suggèrent que le ST peut entraîner une réponse systémique; v) avant d’envisager un ECC pour confirmer l’allergène ou le médicament coupable. Les principales limites du test sont liées à une sensibilité non optimale, en particulier en cas d’allergie médicamenteuse, à la nécessité d’effectuer le test au plus tard 24 heures après l’extraction de l’échantillon et au manque de normalisation entre les laboratoires en termes de procédures, de concentrations et de marqueurs cellulaires.
Le diagnostic d’allergie médiée par les IgE est basé sur les antécédents cliniques, les tests cutanés (ST), la quantification des IgE spécifiques du sérum (IgE) et, si cela est nécessaire et indiqué, les tests de provocation contrôlée (ECC)1,2,3,4,5,6. Cependant, les antécédents cliniques peuvent être peu fiables car il peut y avoir un manque d’informations précises, et les ST et les ECC ne sont pas des procédures sans risque qui peuvent être contre-indiquées chez les sujets présentant des réactions graves mettant la vie en danger 1,2,3,4,5,6 . Ces problèmes, ainsi que le fait que la détermination des sIgE par des immunoessais immunozymatiques validés et commerciaux n’est disponible que pour quelques allergènes et médicaments, ont mis en évidence le rôle important d’autres tests fonctionnels in vitro tels que le test d’activation des basophiles (MTD).
Les basophiles sont des cellules effectrices clés impliquées dans les réactions allergiques médiées par les IgE qui sont activées lors de la réticulation des IgE adjacentes liées aux récepteurs de haute affinité (FcεRI) à la surface des cellules après une exposition à un allergène / médicament. L’activation des basophiles déclenche la dégranulation cellulaire et la libération de médiateurs inflammatoires préformés et nouveaux synthétisés contenus dans les granules de sécrétion intracytoplasmique 7,8,9. La MTD est une méthode in vitro qui tente d’imiter l’activation des basophiles en présence d’un stimulus (allergène ou médicament) et détermine les changements dans l’expression des marqueurs d’activation des basophiles par cytométriede flux 7,10. Il existe différentes stratégies pour identifier les basophiles (IgE+, CCR3+, CRTH2+, CD203c+) et pour mesurer l’activation cellulaire (principalement la régulation positive de CD63 et CD203c) à l’aide de combinaisons d’anticorps marqués au fluorochrome 7,10. CD63, le meilleur marqueur d’activationvalidé cliniquement 11,12,13,14, est une protéine membranaire ancrée aux granules sécrétoires contenant de l’histamine qui, après activation cellulaire et fusion des granules avec la membrane, est exprimée à la surface des basophiles 15,16,17,18,19,20,21 . CD203c est un marqueur de surface qui est exprimé de manière constitutive sur les basophiles et régulé à la hausse après stimulation FcεRI, qui a également montré des résultats fiables dans BAT 15,22,23,24,25. En outre, il semble co-exprimer avec CD6326.
Au cours des dernières décennies, la MTD s’est révélée utile dans le diagnostic des réactions allergiques médiées par les IgE induites par différents déclencheurs tels que les médicaments, les aliments ou les inhalants, ainsi que dans certaines formes d’urticaire chronique, comme décrit ci-dessous. Cependant, la position de cette technique dans les algorithmes de diagnostic est très variable et mal déterminée.
Hypersensibilité aux médicaments
La MTD s’est révélée utile comme test complémentaire pour certains médicaments et patients, en particulier pour ceux qui présentent des réactions graves en raison du fait que la valeur diagnostique du ST n’est pas bien établie pour la plupart des médicaments, car elle est validée et normalisée pour un nombre limité de médicaments27,28,29,30. De plus, la quantification des IgE n’est disponible que pour un nombre limité de médicaments, avec une sensibilité inférieure à ST 27,28,29,30,31,32. Par conséquent, le diagnostic d’hypersensibilité aux médicaments repose généralement sur un test de provocation médicamenteuse, qui peut être contre-indiqué chez les sujets présentant des réactions graves mettant la vieen danger 33.
Des résultats prometteurs ont été rapportés pour l’utilisation de la MTD chez certains patients signalant des réactions d’hypersensibilité immédiate à différents médicaments tels que les bêtalactamines (BL)20,34,35,36,37,38,39, les agents bloquants neuromusculaires (AMNM)19,22,40,41,42, 43,44,45, fluoroquinolones (FQs)46,47,48,49, pyrazolones 50,51,52, produits de contraste radioactif (RCM)53,54,55,56 et composés du platine57,58,59 . On a signalé que la sensibilité et la spécificité des MTD se situaient entre 51,7 et 66,9 % et entre 89,2 et 97,8 %, respectivement; et les valeurs prédictives positives et négatives sont décrites comme variant entre 93,4 % et 66,3 %, respectivement27,31. De plus, la MTD a été proposée comme biomarqueur prédictif des réactions paroxystiques lors de la désensibilisation avec les composés du platine, car l’expression de CD203c est augmentée par rapport à CD63 chez les patients présentant un risque élevé d’effets indésirables lors de la désensibilisation au médicament57.
Il est à noter que la MTD n’est utile dans l’hypersensibilité aux médicaments que lorsque la réaction implique une dégranulation des basophiles; Par conséquent, il n’est pas utile dans les réactions résultant de l’inhibition enzymatique de la cyclooxygénase 142.
Allergie alimentaire
La MTD est apparue comme un outil de diagnostic potentiel pour les allergies alimentaires parce que la détermination des IgE sériques dans l’extrait d’allergène entier ou dans un allergène unique est souvent ambiguë, nécessitant une provocation alimentaire orale pour confirmer le diagnostic, ce qui, à l’instar de l’hypersensibilité aux médicaments, est une procédure coûteuse et non sans risque60. Plusieurs études ont montré des résultats pertinents avec le lait de vache 61,62, l’œuf61,63, le blé 64,65,66,67,68, l’arachide 63,69,70,71,72, la noisette 73,74,75,76 ,77, crustacés78, pêches 79,80,81, pomme21, céleri et carotte82,83.
La principale valeur ajoutée des MTD dans le diagnostic des allergies alimentaires par rapport aux ST et aux IgE dans le sérum est qu’elles présentent une spécificité plus élevée et une sensibilité similaire. Ainsi, la MTD est un outil utile pour différencier les patients cliniquement allergiques des sujets sensibilisés, mais tolérants, qui ont à la fois une spécificité élevée (75-100%) et une sensibilité (77-98%)63,69,84. Les valeurs de sensibilité et de spécificité dépendent de l’allergène et d’autres facteurs comme les phénotypes (p. ex. syndrome d’allergie orale par rapport à l’anaphylaxie), l’âge et les profils de sensibilisation liés à la géographie63,85.
Les MTD utilisant des composants allergènes uniques peuvent potentiellement améliorer la précision diagnostique de certains allergènes alimentaires61,80. Des études utilisent des protéines d’entreposage des semences (p. ex. Ara h 1, Ara h 2, Ara h 3 et Ara h 6 de l’arachide)86; protéines de transfert lipidique (p. ex. Pru p 3 de la pêche et Ara h 9 de l’arachide)80,86; et les homologues Bet v 1 (par exemple, Ara h 8 de l’arachide)87. D’autres utilités potentielles sont liées à l’identification de l’allergène coupable dans les cas de syndrome d’allergie pollen-alimentaire 21,87,88, d’allergie à la viande rouge 89 ou d’anaphylaxie induite par l’exercice alimentaire66.
Fait intéressant, la MTD peut fournir des informations sur la gravité et le seuil des réactions allergiques puisque les patients présentant des réactions plus graves présentent une plus grande proportion de basophiles activés, comme l’ont observé des études sur des patients allergiques aux arachides et au lait de vache 84,90,91; et les patients réagissant à des traces de l’allergène montrent une plus grande sensibilité aux basophiles 84,90,92. Ces données suggèrent que la MTD pourrait être utile pour identifier les patients allergiques à haut risque qui nécessitent un suivi plus étroit et une éducation plus intensive93. En outre, il a été rapporté que les MTD peuvent prédire les réponses de provocation alimentaire 70,91,92,94 et les seuils de réactivité90,95 pour aider à déterminer quand les aliments peuvent être (ré)introduits en toute sécurité 84. Cependant, ces résultats sont controversés dans certaines études 63,96 et d’autres recherches sont nécessaires.
D’autre part, la MTD a été utilisée pour surveiller la résolution de l’allergie alimentaire, soit naturellement, soit sous traitement immunomodulateur, au fil du temps, qui jusqu’à présent n’a été évaluée que par provocation alimentaire orale, avec les risques et les coûts associés 84,97,98,99,100,101,102,103,104,105,106 ,107 108. De plus, il a également été utilisé pour surveiller l’effet de l’omalizumab dans les allergies alimentaires, car l’activation des basophiles diminue pendant le traitement par omalizumab, mais elle augmente après l’arrêt du traitement109.
Allergie aux inhalants
La MTD est rarement bénéfique dans l’allergie aux inhalants, car le diagnostic peut être systématiquement établi par quantification des IgE et ST. Cependant, dans les cas de rhinite allergique locale (taux indétectables d’IgE et de ST négatifs avec tests de provocation nasale positifs), la MTD a permis le diagnostic dans 50% des cas110. Il a également été rapporté une corrélation entre la sensibilité basophile et la réponse aux tests de provocation nasale / bronchique, ainsi qu’entre la gravité de l’asthme et l’efficacité du traitement par omalizumab111,112.
La MTD a également été utilisée pour surveiller l’immunothérapie allergénique pour les acariens et les pollens, car la sensibilité aux basophiles diminue pendant l’immunothérapie, probablement en raison de l’interférence des anticorps IgGbloquants 113,114,115,116,117.
Allergie au venin d’hyménoptère
Le diagnostic de l’allergie au venin d’hyménoptère est systématiquement basé sur les ST et les IgE sériques. La MTD a montré une sensibilité élevée (85-100%) et une spécificité (83-100%) et elle a été rapportée utile dans les cas qui donnent des résultats équivoques ou chez les patients ayant des antécédents cliniques suggestifs d’allergie au venin mais des IgE indétectables et ST118,119 négatives. Cependant, les MTD ne semblent pas être prédictives de la gravité de ces réactions 120 121.
Jusqu’à 60% des patients présentent des IgE à la fois au venin de guêpe et d’abeille, et l’identification de l’allergène dominant est cruciale pour un traitement immunothérapeutique adéquat. Dans ces cas, la MTD a été jugée utile dans l’identification de l’allergène dominant119 122 123 124. Bien que les sIgE aux principaux allergènes des venins d’abeilles et de guêpes puissent réduire l’utilité de la MTD chez les patients présentant une double positivité aux deux venins, elle fournit des informations utiles principalement chez les sujets présentant des résultats négatifs dans les déterminations d’IgE123.
Certaines études suggèrent que la MTD pourrait être utile comme biomarqueur prédictif des effets secondaires pendant la phase d’accumulation de l’immunothérapie au venin, car il a été rapporté que cette option de traitement diminuait la sensibilité aux basophiles. Cependant, la réactivité ne diminue pas et cette utilité BAT est aujourd’hui controversée13 120 125 126 127 128 129 130.
Urticaire et œdème de Quincke
Un sous-ensemble de patients atteints d’urticaire chronique ont une physiopathologie auto-inmanne, due aux autoanticorps IgE dirigés contre les autoallergènes et aux autoanticorps IgG qui ciblent les complexes FcεRI ou IgE-FcεRI présents à la surface des mastocytes 131 132. En pratique clinique, le diagnostic de ce type d’urticaire chronique repose sur le ST autologue positif, qui présente un risque d’infection accidentelle. La MTD a été proposée comme test in vitro pour le diagnostic et le suivi des patients suspectés d’urticaire chronique. L’expression de CD63 et CD203c à la surface des basophiles a été signalée comme étant augmentée après stimulation avec des sérums de patients atteints d’urticaire chronique, montrant la détection d’autoanticorps actifs 133,134,135,136,137. Récemment, il a été rapporté que les patients atteints de MTD positive éprouvent souvent l’état pathologique le plus actif, évalué par le score d’activité de l’urticaire, et nécessitent des doses plus élevées d’antihistaminiques associés à des traitements de troisième intention (cyclosporine A ou omalizumab), par rapport à ceux ayant une MTD138 négative.
La MTD est un test de diagnostic in vitro complémentaire pour l’évaluation des réactions allergiques médiées par les IgE qui s’est avéré utile dans le diagnostic des réactions induites par différents déclencheurs tels que les médicaments, les aliments ou les inhalants, ainsi que dans certaines formes d’urticaire chronique.
