Er wordt een protocol gepresenteerd om te evalueren of kleine EV’s (sEV’s) geïsoleerd uit placenta-explantaten gekweekt onder hypoxische omstandigheden (modellering van één aspect van pre-eclampsie) de bloed-hersenbarrière verstoren bij niet-zwangere volwassen vrouwelijke muizen.
Cerebrovasculaire complicaties, waaronder hersenoedeem en ischemische en hemorragische beroerte, vormen de belangrijkste oorzaak van moedersterfte geassocieerd met pre-eclampsie. De onderliggende mechanismen van deze cerebrovasculaire complicaties blijven onduidelijk. Ze zijn echter gekoppeld aan placentadisfunctie en verstoring van de bloed-hersenbarrière (BBB). Niettemin wordt het verband tussen deze twee verre organen nog steeds vastgesteld. Toenemend bewijs suggereert dat de placenta signaalmoleculen, waaronder extracellulaire blaasjes, afgeeft aan de maternale circulatie. Extracellulaire blaasjes worden gecategoriseerd op basis van hun grootte, waarbij kleine extracellulaire blaasjes (sEV’s kleiner dan 200 nm in diameter) worden beschouwd als kritische signaaldeeltjes in zowel fysiologische als pathologische omstandigheden. Bij pre-eclampsie is er een verhoogd aantal circulerende sEV’s in de maternale circulatie, waarvan de signaalfunctie niet goed wordt begrepen. Placenta-sEV’s die vrijkomen bij pre-eclampsie of van normale zwangerschapsplacenta’s die worden blootgesteld aan hypoxie, veroorzaken endotheeldisfunctie in de hersenen en verstoring van de BBB. In dit protocol beoordelen we of sEV’s geïsoleerd uit placenta-explantaten gekweekt onder hypoxische omstandigheden (modellering van een aspect van pre-eclampsie) de BBB in vivo verstoren.
Ongeveer 70% van de maternale sterfte als gevolg van pre-eclampsie, een hypertensief zwangerschapssyndroom dat wordt gekenmerkt door verminderde placentatieprocessen, maternale systemische endotheeldisfunctie en, in ernstige gevallen, multi-orgaanfalen 1,2, wordt geassocieerd met acute cerebrovasculaire complicaties 3,4. De meeste moedersterfte komt voor in lage- en middeninkomenslanden5. De onderliggende mechanismen zijn echter nog steeds onduidelijk, ondanks de klinische en epidemiologische relevantie van cerebrovasculaire complicaties geassocieerd met pre-eclampsie.
Aan de andere kant zijn extracellulaire vesikels (EV’s) (diameter ~30-400 nm) essentiële mediatoren van intercellulaire communicatie tussen weefsels en organen,inclusief maternale en placenta-interactie6. Naast eiwitten en lipiden op het uitwendige oppervlak, vervoeren EV’s lading naar binnen (eiwitten, RNA en lipiden). EV’s kunnen worden onderverdeeld in (1) exosomen (diameter ~50-150 nm, ook wel kleine EV’s (sEV’s) genoemd), (2) middelgrote/grote EV’s en (3) apoptotische lichamen, die verschillen in grootte, biogenese, inhoud en potentiële signaalfunctie. De samenstelling van EV’s wordt bepaald door de cellen waaruit ze afkomstig zijn en het ziektetype7. Syncytiotrofoblast-afgeleide EV’s brengen placenta alkalische fosfatase (PLAP)8,9 tot expressie, die van placentae afgeleide circulerende kleine EV’s (PDsEV’s) tijdens de zwangerschap detecteert. PLAP helpt ook bij het onderscheiden van veranderingen in de lading van de PD’sEV’s en hun effecten bij pre-eclampsie versus normotensieve zwangerschappen 10,11,12,13,14,15.
De placenta is erkend als de noodzakelijke component in de pathofysiologie van pre-eclampsie16 of cerebrale complicaties geassocieerd met deze ziekte 17,18,19. Het is echter onbekend hoe dit verre orgaan veranderingen in de hersencirculatie kan veroorzaken. Aangezien sEV’s een cruciale rol spelen in cel-tot-celcommunicatie vanwege hun vermogen om bioactieve componenten over te dragen van donor- naar ontvangercellen 6,20,21, heeft een groeiend aantal onderzoeken placenta-sEV’s in verband gebracht met het ontstaan van maternale endotheeldisfunctie 21,22,23,24, inclusief endotheelcellen in de hersenen 25,26bij vrouwen met pre-eclampsie. Het compromitteren van de endotheelfunctie van de hersenen kan dus leiden tot verstoring van de bloed-hersenbarrière (BBB), een cruciaal onderdeel bij cerebrovasculaire complicaties geassocieerd met pre-eclampsie3,27.
Niettemin meldden preklinische bevindingen met behulp van cerebrale vaten van ratten die waren blootgesteld aan serum van vrouwen met pre-eclampsie28 of menselijke endotheelcellen van de hersenen die waren blootgesteld aan plasma van vrouwen met pre-eclampsie29 dat circulerende factor(en) verstoring van de BBB veroorzaken. Ondanks verschillende kandidaten met het potentieel om de BBB te schaden die aanwezig is in de maternale circulatie tijdens pre-eclampsie, zoals verhoogde niveaus van pro-inflammatoire cytokines (d.w.z. tumornecrosefactor)18,28 of vasculaire regulatoren (d.w.z. vasculaire endotheliale groeifactor (VEGF))29,30,31, of oxidatieve moleculen zoals geoxideerde lipoproteïnen (oxo-LDL)32,33, onder andere 34, geen van hen brengt een directe verbinding tot stand tussen de placenta en de BBB. Onlangs hebben sEV’s geïsoleerd uit hypoxische placenta’s aangetoond dat ze de BBB kunnen verstoren bij niet-zwangere vrouwelijke muizen25. Aangezien placenta-sEV’s de meeste van de genoemde circulerende factoren kunnen dragen met het vermogen om de BBB te verstoren, worden sEV’s beschouwd als geschikte kandidaten om de gewonde placenta te verbinden, de drager te zijn van schadelijke circulerende factoren en de BBB te verstoren bij pre-eclampsie.
Dit protocol stelt ons in staat om te onderzoeken of sEV’s geïsoleerd uit placenta-explantaten gekweekt onder hypoxische omstandigheden de BBB kunnen verstoren bij niet-zwangere vrouwelijke muizen als een proxy voor het begrijpen van de pathofysiologie van cerebrale complicaties tijdens pre-eclampsie.
Deze studie onthult nieuwe inzichten in mogelijke schade als gevolg van sEV’s geïsoleerd uit placenta-explantaten gekweekt in hypoxische omstandigheden op de verstoring van de bloed-hersenbarrière van knaagdieren. Het pathologische mechanisme omvat een vermindering van CLND-5 in het achterste hersengebied25.
