Lunge CT Segmentering at identificere konsolideringer og malet glas områder for kvantitative assesment af SARS-CoV lungebetændelse

Summary

Formålet med denne protokol er at give en tidseffektiv måde at segmentere mængder af interesse på ct-scanninger med høj opløsning, der kan bruges til yderligere radiomikanalyse.

Abstract

Segmentering er en kompleks opgave, som radiologer og forskere står over for, efterhånden som radiomik og maskinlæring vokser i potentiale. Processen kan enten være automatisk, halvautomatisk eller manuel, den første er ofte ikke tilstrækkelig præcis eller let reproducerbar, og den sidste er alt for tidskrævende, når der involveres store distrikter med høj opløsning erhvervelser.

En høj opløsning CT af brystet er sammensat af hundredvis af billeder, og det gør den manuelle tilgang alt for tidskrævende. Desuden kræver parenkymale ændringer, at der skal foretages en ekspertvurdering ud fra det normale udseende. Således er en halvautomatisk tilgang til segmenteringsprocessen, så godt vi ved, den mest velegnede, når de segmenterer lungebetændelser, især når deres funktioner stadig er ukendte.

For de undersøgelser, der udføres i vores institut om billeddannelse af COVID-19, vedtog vi 3D Slicer, et freeware software produceret af Harvard University, og kombineret tærsklen med pensel instrumenter til at opnå hurtig og præcis segmentering af beædte lunge, jorden glas opacities, og konsolideringer. Når denne metode står over for komplekse sager, kræver den stadig en betydelig mængde tid til korrekt manuelle justeringer, men giver et yderst effektivt middel til at definere segmenter, der skal anvendes til yderligere analyse, såsom beregning af procentdelen af de berørte lungeparenkym eller teksturanalyse af jordens glasområder.

Introduction

I indeværende år, verden står over for en sundheds-nødsituation, pandemi forårsaget af romanen Coronavirus, Sars-CoV2. Selv om, indtil den nuværende dato, mange aspekter vedrørende fysiopatologi af COVID-19 infektion er stadig uklart, det deler flere egenskaber med sine "forfædre" SARS1 og MERS. Især er det blevet bevist, at virion spike proteiner interagerer med Angiotensin Konvertering enzym Type-2, en receptor godt repræsenteret på alveolær endotelceller, men allestedsnærværende i den menneskelige organisme, og dermed har potentiale til at give systemiske symptomer1.

Til diagnosticering er den nuværende standard den omvendte transskription-polymerasekædereaktion (rt-PCR), en test udført på svælgsvaberprøver. Selv om radiologisk billeddannelse ikke er officielt anerkendt i den diagnostiske vej til påvisning af sygdommen, viste højopløsningstesttomografi (HRCT) sig at være et værdifuldt hjælpemiddel til den kliniske og epidemiologiske behandling af berørte patienter på grund af rt-PCR's relativt lave følsomhed, den nuværende knaphed på specialiserede laboratorier og de nødvendige reagenser og den høje operatørafhængighed.

Radiological Society of North America (RSNA) udgivet en konsensus erklæring, godkendt af selskabet thorax radiologi og American College of Radiology (ACR), der klassificerer CT udseende COVID19 i fire kategorier for at standardisere rapportering, dividere den interstitielle lungebetændelse mønstre i "typiske", "atypiske", "ubestemt", og "negativ"2.

Den "typiske" mønster er kendetegnet ved tilstedeværelsen af rundformede Ground Glass Opacities (GGO), normalt med en sub-pleural placering på dorsale basal segmenter. GGO kan være forbundet med "Crazy Bane" områder af fortykket septa, eller andre tegn på at organisere lungebetændelse. Det "ubestemte" mønster er karakteriseret ved fraværet af de "typiske" mønster fund, med diffuse GGO områder med en perihilar fordeling, med eller uden konsoliderede områder. Det "atypiske" mønster er karakteriseret ved enten fraværet af de "typiske" eller "ubestemte" tegn, og tilstedeværelsen af lobar konsolideringer, "træ i opløbet", glat fortykkelse af septa og pleural effusion; i denne præsentation kan der ikke påvises nogen GGO. Det "negative" mønster er karakteriseret ved fraværet af de førnævnte patologiske fund.

Ifølge litteraturen kan nogle patienter præsentere en høj klinisk mistænkt for COVID-19 understøttet af epidemiologiske kriterier og billeddannelse fund med negativ rt-PCR3,4. På den anden side er det blevet rapporteret, at patienter med en positiv rt-PCR og suggestive kliniske fund, ikke præsenterer patologiske resultater på HRCT5.

I dag er det af afgørende interesse for det videnskabelige samfund at anvende billedanalyseteknikker, når man kvantitativt undersøger sygdommens karakteristika. En nylig undersøgelse har anvendt automatiseret segmentering teknik lunge parenkym at identificere procentdelen af beædnet lunge hos patienter ramt af COVID-19, korrelere denne værdi med prognosen, og viser, at patienter med en mere alvorlig lunge involvering præsenteret større risiko for at blive optaget i intensivafdelingen (ICU), og har dårligere resultater6.

Segmenteringen er kontureringen af interesseområder (ROI'er) i en volumen, der er erhvervet ved hjælp af en billedteknik, f.eks. Denne aktivitet kan udføres gennem tre metoder: manuel, halvautomatisk og automatisk. Manuel segmentering består takket være en uddannet radiologs erfaring i mærkning af voxels, der tilhører det patologiske område. De største ulemper ved denne metode er den store mængde tid, der kræves, og det forhold, at den er operatørafhængig.

Halvautomatiske metoder gør det muligt at fremskynde segmenteringen, da operatøren kan ændre en segmenteringsmaske, der er opnået ved hjælp af de klassiske billedbehandlingsmetoder (f.eks. tærskel for pixelintensitet, klyngedannelse osv.). Disse teknikker er dog ikke lette at implementere i klinisk praksis, da de kræver omfattende manuel indgriben i de mest komplicerede tilfælde18.

Automatiske segmenteringsmetoder, der i øjeblikket er af begrænset anvendelse, anvender kunstig intelligens til at opnå ROI'er. En nylig undersøgelse har navnlig til formål at anvende automatisk segmentering i kvantificeringen af jorden glasområder hos patienter, der lider af COVID-19 interstitiel lungebetændelse19. Definitionen af en segmenteringsprotokol for de patologiske områder på HRCT-billederne er det virkelige første skridt mod den efterfølgende radiomikanalyse for at identificere funktioner, der kan bidrage til yderligere at forstå sygdommens fysiopatologi og tjene som en nøjagtig prognostisk faktor, der potentielt kan påvirke behandlingen.

Protocol

Denne protokol følger retningslinjerne fra den institutionelle humane forskningsetiske komité.

1. Download af DICOM-billederne

  1. Download DICOM-billederne, og overfør dem på den arbejdsstation, der er dedikeret til segmenteringen, med 3D-softwaren installeret. Hvis du planlægger at arbejde på en personlig computer, anonymisere DICOM data.

2. Import af HRCT-undersøgelsen af 3D Slicer-softwaren

  1. Vælg Indlæs DICOM-data på skærmen til åbning af softwaren (svarende til afsnittet Velkommen til udsnit i rullemenuen). Du kan også vælge DCM-ikonet i øverste venstre hjørne af værktøjslinjen.
  2. Vælg Importér i øverste venstre hjørne af panelet DICOM Browser , og vælg derefter placeringen af HRCT-undersøgelsen. Vælg mappen med DCM-aftrykkene, og vælg derefter Importér.
  3. Nu, hvor HRCT-undersøgelsen er blevet importeret, skal du klikke på knappen Belastning.

3. Oprettelse af segmenter

  1. Opret segmenter i sektionen Segmenteringer i rullemenuen eller direkte i underafsnittet Segmenteditor, der findes i rullemenuens segmenteringssektion eller igen som et dedikeret ikon på værktøjslinjen.
  2. Vælg HRCT-undersøgelsen i rullemenuen ud for Master Volume.
  3. Vælg Tilføj, og opret tre nye segmenter, som automatisk får navnet Segment 1, 2 og 3. Dobbeltklik på hver enkelt og omdøb dem henholdsvis "tlv" (Total Lung Parenchyma), "ggo" (Ground Glass Opacities) og "ulemper" (Konsolideringer). Hvis der i HRCT undersøgelse yderligere patologiske resultater sameksistere, såsom pleural effusion, lungekræft, fibrotiske områder og så videre, skabe yderligere segmenter. Det samme gælder for artefakter.

4. Definition af TLP-segmentet

BEMÆRK: En nøjagtig definition af TLP-segmentet er grundlæggende, da det vil blive brugt til at maskere HRCT under definitionen af GGO- og cd'erne.

  1. Som et første trin skal du vælge tærskelinstrumentet i sektionen Segmenteditor, når du har valgt TLP-segmentet.
  2. Sæt en tærskel stor nok til at omfatte både den sunde lunge parenkym og jorden glas opacities. Ved hjælp af de værdier, der er rapporteret i den videnskabelige litteratur med de nødvendige justeringer for at overholde lærebogsdefinitionen af GGO, konstaterede vi, at fastsættelsen af tærsklen mellem -1000 HU og -250 HU fungerergodt 6,14,15,16,17. Vælg derefter Anvend.
  3. Det således definerede segment vil omfatte både luften inde i lungerne og luften uden for brystet (dvs. luften uden for patienten). For at isolere lungeparenkym, skal du bruge værktøjet Øer, der findes i segmenteditoren, og vælg Bevar valgt ø. Venstre klik inde i brystet, så noget uden for brystet vil blive udelukket fra segmentet.
  4. Derefter skal du medtage ethvert konsolideringsområde i TLP-segmentet. Konsolideringerne har normalt dæmpningsværdier (Hounsfield Units: HU) svarende til dem, der gælder for blødt væv, der udgør brystvæggen og mediastinum. Dette er grunden til, at tærskelinstrumentet ikke kan bruges til dette formål, og konsolideringerne skal tilføjes manuelt ved hjælp af sakse- og malingsværktøjerne, der findes i segmenteditoren.
    1. Brug sakseværktøjet til at tilføje dele af lungeparenkym til segmentet. Vælg handlingen Udfyld indvendig, vælg Fri form som form og Symmetrisk som udsnitssnittet. Vælg en tykkelse, der passer til konsolideringens størrelse (f.eks. fra 3 til 20 mm). og derefter begynde gradvist at tilføje små portioner af TLP, indtil konsolideringen er helt inkluderet.
    2. Paint-værktøjet er nemmere og nogle gange hurtigere at bruge, men det kan være mindre præcist, afhængigt af hvilket område der skal medtages. Når indstillingen Sphere Brush er aktiv, kan dette værktøj tilføje 3D-dele af lungeparenkym til segmentet. Størrelsen af børsten kan nemt ændres.
    3. Når konsolideringerne føjes til TLP-segmentet, medtages ikke dele af brystvæggen eller mediastinum. Dette kan vise sig vanskeligt som hos patienter ramt af COVID19 konsolideringer støder op til den bageste thorax væggen er ret almindelige. Brug værktøjet Slet til at rette segmenteringen, eller vælg Fortryd for at kassere den sidste handling.
    4. På dette stadium skal du bruge udjævningsalgoritmen igen til at slette små mangler, der kunne være opstået under den manuelle del af segmenteringen. Nu er TLP-segmentdefinitionen afsluttet.

5. Definition af GGO-segmentet

  1. Hvis du vil definere GGO-segmentet, skal du bruge tærskelværktøjet.
    1. Vælg GGO-segmentet.
    2. Sæt tærsklen mellem -750 HU og -150 HU.
    3. Før du vælger Anvend, skal du gå til sektionen Maskering lige nedenunder og vælge TLP i rullemenuen Redigerbart område. Vælg Ingen i rullemenuen Overskriv andet segment. Dette er yderst vigtigt, da, hvis sprunget, når defineret GGO segment, også TLP segmentet ville blive ændret. Dette skal undgås, da TLP-segmentet er den maske, som alle de andre segmenter er defineret på.
    4. Vælg nu Anvend.
  2. På dette tidspunkt vil det sandsynligvis være nødvendigt at bruge udjævningsalgoritmen igen for at udelukke alle de små fartøjer og de fysiologiske interstitielle elementer, der har en tæthed svarende til jordens glas fra segmentet. Start med en kernestørrelse på 3 mm og forøg den om nødvendigt gradvist til maksimalt 6-7 mm. Bemærk, at forøgelse af kernestørrelsen for meget kan bestemme tabet af små områder af malet glas, som ville blive efterladt uden for GGO-segmentet. På grund af dette, udjævning algoritme bør anvendes med den rette forsigtighed.
  3. I dette tilfælde skal du også anvende nødvendige rettelser med værktøjet Paint, Scissors and Erase. I denne fase skal du altid huske at holde TLP-masken aktiv for at undgå at inkludere dele af brystvæggen ved siden af "malet glas" områder inde i segmentet. Dette skyldes, at hvis de fejlagtigt segmenteres, vil de automatisk blive udelukket.
  4. Når du opnår dette segment, være meget opmærksomme på den mulige tilstedeværelse af artefakter genereret af bevægelser af arne og mellemgulv (dette sker, hvis patienten ikke kunne holde vejret under eksamen). Hvis de findes og tidligere er inkluderet i TLP-segmentet, skal du fjerne disse artefakter fra segmentet "Malet glas", f.eks. Afhængigt af undersøgelsens mål kan artefakterne segmenteres separat ved hjælp af et dedikeret segment eller inkluderet i TLP-segmentet og udelukkes fra andre segmenter. Nu er GGO-segmentet defineret.

6. Definition af cd'er-segmentet

  1. Hvis du vil definere cd-segmentet, skal du fortsætte på samme måde som for at definere GGO-segmentet.
    1. Vælg cd-segmentet. Sørg for at arbejde med TLP maskering altid aktiv.
    2. Sæt en passende tærskel. For konsolideringer varierer rækkevidden omtrent fra -150 HU til 100 HU.
    3. Anvend udjævningsalgoritmen, der varierer kernestørrelsen efter behov.
    4. Brug en kombination af værktøjerne Saks, Slet, Maling og Øer for at holde inde i segmentet kun de reelle konsolideringer, undtagen store fartøjer, pleural effusions, artefakter, disventilatoriske bånd og andre ikke COVID-relaterede læsioner.
    5. Hvis det er nødvendigt, skal du anvende en endelig udjævning.

7. Lagring af segmenter

  1. Gem segmenter som en ".nrrd"-fil, eller konverter til binære etikettilknytninger fra modulet "Data".

8. Udvinding af volumener fra de definerede segmenter.

  1. Fra modulet Segmentstatistik skal du få en tabel med oplysninger om segmenters volumen og overflader.

Representative Results

Den foreslåede metode er blevet raffineret gennem forsøg og fejl, teste det på 117 patienter ramt af COVID-19 lungebetændelse med en positiv rt-PCR test.

Efter en kort indlæringskurve kan den tid, der er nødvendig for at opnå segmenterne, variere fra 5 til 15 minutter, afhængigt af præsentationsmønsteret.

Som vist i figur 1giver metoden præcise segmenter: Dette kan observeres ved at bemærke den nøjagtige korrespondance med HRCT. 3D-gengivelsen hjælper med at vurdere korrespondancen og gennemgå hurtigt segmenteringsresultaterne. Der kan opnås en kvantitativ vurdering af mængden af påvirket lungeparenkym for at kopiere de rapporterede resultater fra Colombi et al.6 og Lanza et al.13.

Discussion

Segmentering udgør et grundlæggende skridt i forbindelse med udførelse af moderne kvantitative radiologiundersøgelser og er nødvendig for at anvende radiomik- eller teksturanalyseteknikker. Patologiske fund i lungerne repræsenterer en af de mest udfordrende at segmentere, for manglen på definerede anatomiske grænser og en lille forskel i dæmpningsværdi sammenlignet med de sunde områder.

Kildebillederne skal præsentere med et minimum af artefakter, hvis det er muligt, især på de patologiske områder, og det er nogle gange svært at opnå, når man studerer en sygdom, der kompromitterer åndehold; Derfor kan forskerne overveje at udelukke kompromitterede HRCT'er eller definere et segment dedikeret til artefakter, der skal elimineres fra yderligere analyse.

Det er muligt at installere en udvidelse til 3D Slicer, kaldet "chest imaging platform"11, som giver mulighed for hurtigere, mere automatiserede operationer på lungesegmenter, med særlig interesse på beædnet lunge. Det blev besluttet ikke at anvende denne metode, da det kræver omfattende manuel indgriben, når GGO og konsolideringer har en sub-pleural distribution, hvilket er tilfældet med den patologi udforsket i dette papir.

Der er rapporteret om en automatisk segmenteringsmetode for interstitielle lungesygdomme12; alligevel krævede denne metode, at der tidligere havde kendskab til de berørte områders karakteristika. Den teknik, der foreslås i denne undersøgelse, repræsenterer en let at lære og reproducerbar tilgang til segmentering af patologiske fund af lungerne, hvis egenskaber kan give fremtidige midler til automatisk at segmentere virale interstitielle lungesygdomme og repræsentere nøjagtige prognostiske faktorer.

Den foreslåede segmenteringsmetode har visse begrænsninger.

Først og fremmest, downloade DICOM billeder fra arbejdsstationer er en proces, der kan kræve en variabel mængde tid, og dermed gøre det for et stort antal patienter kan være en besværlig arbejde. Enhver potentiel anvendelse af denne segmenteringsmetode på klinikken praksis skal overveje dette kritiske spørgsmål, indtil det punkt, hvor segmentering plugins bliver bredt tilgængelige på PACS platforme.

For det andet kan segmenteringen af COVID-19-relaterede patologiske områder være kompliceret hos patienter med samtidig kronisk lungesygdom (f.eks. lungekræft, lungefibrose osv.), hvis radiologiske fund består i områder med samme tætheder som dem, der er typiske for COVID-19-mønstre. Den samme bekymring bør overvejes i CT-scanninger med respiratoriske artefakter. Disse artefakter er ret almindelige hos patienter med COVID-19 infektion, er almindeligt relateret til dyspnø og respirationssvigt, især hos gamle / midaldrende patienter.

Desuden patienter med en alvorlig interstitiel lungebetændelse (karakteriseret ved mange lungekonsolideringer og skøre brolægning opaciteter) kræver en mere omfattende manuel segmentering og dermed en enorm mængde tid. Generelt, jo højere sværhedsgraden af den interstitielle lungebetændelse, jo mere omfattende manuel segmentering kræves, jo længere segmentering tid.

Den grad af præcision, der tilbydes af avancerede evalueringsteknikker som dem, der foreslås af Lanza og Colombi, kan imidlertid tilføje begrænsede oplysninger om patienter med allerede alvorlige lungesygdomme sammenlignet med standard kliniske og radiologiske evalueringer20.

Endelig skal det bemærkes, at enhver radiolog, der ikke har nogen erfaring med 3D-slicer har brug for en tilstrækkelig træningstid, da det ikke er en intuitiv software og kræver lidt tid til at blive mestret selv i sin basilar funktioner.
