人源样本的采集已获天津医科大学总医院医学伦理委员会批准。人源静脉血的采集严格遵循中国国家卫生健康委员会发布的WS/T 661-2020《静脉血液标本采集指南》。简要而言，在取得知情同意后，从健康个体的前臂静脉采血，并以1:9的比例使用3.2%枸橼酸钠抗凝剂混匀样本。当仅采集枸橼酸钠抗凝标本时，弃去第一管血液。样本采集后0.5小时内即开始处理流程。在取得知情同意后，招募成年健康受试者进行样本采集。患者的排除标准为：（1）近期发生血管内血栓，（2）肝肾功能障碍，（3）高血压、高脂血症、糖尿病及其他慢性疾病，（4）正在接受阿司匹林或抗凝治疗，（5）处于月经期或妊娠期。
1. 富含细胞外囊泡血浆的分离
<ol>
	<li>在室温下以 120 × g 离心 20 分钟，去除血细胞。上清液为富血小板血浆。然后用移液器将上清液的上半部分转移至新的离心管中。</li>
	<li>在室温下以 1500 × g 离心 20 分钟，去除血小板。上清液为乏血小板血浆。然后将上清液的上半部分转移至新的离心管中。</li>
	<li>在室温下以 13000 × g 离心 2 分钟，去除细胞碎片。上清液为富含细胞外囊泡（EV）的血浆。然后将上清液的上半部分转移至新的离心管中，用于后续检测。</li>
</ol>
2. 利用分析仪检测样本中的EV-ACT
<ol>
	<li>开启凝血分析仪（参见材料表），并将仪器预热至 37 °C。安装一次性探针和检测杯，然后启动仪器的质量控制程序。 
	注意：当屏幕上显示的黏滞阻力信号值呈一条直线时，表明检测未受干扰，分析仪状态的质量控制合格。自检合格后，可开始检测程序。</li>
	<li>将样本信息输入系统。随后，将 200 µL 富含细胞外囊泡（EV）的血浆转移至检测杯中，再加入 20 mM 的氯化钙溶液 170 µL。点击启动按钮。检测杯内的磁力搅拌棒将充分混匀样本与氯化钙。关闭盖子，探针将开始检测样本阻力的变化。 
	&#8203;注意：检测完成后，分析仪将自动提示&#34;检测完成&#34;。EV-ACT 时间由系统显示并记录，结果以时间单位&#34;秒&#34;表示。丢弃探针和检测杯。</li>

3. 基于流式细胞术的EV富集血浆样本质量控制
<ol>
	<li>EVs 最初是根据其大小（0.1–1 µm）被识别的。预先调整流式细胞仪的参数并设定 &#34;门控&#34; 使用市售聚苯乙烯微球（0.5、0.9 和 3.0 µm）进行细胞外囊泡（EV）的分离（参见 材料表). 
	注意：微珠混合物包含不同直径的荧光微球，用于覆盖微囊泡（0.5 和 0.9 µm）和血小板（0.9 µm 和 3 µm）的粒径范围。</li>

血栓形成由高凝状态引起，在多种疾病中发挥重要作用，包括脑外伤1、子痫前期2、肿瘤3以及骨折患者4。高凝状态的机制复杂，近年来研究重点集中在细胞外囊泡（EV）在凝血功能障碍中的作用。EV 是具有双层结构的囊泡样结构，从细胞膜脱落，直径范围为 10 nm 至 1000 nm，与多种疾病过程相关，尤其是凝血障碍5。多项研究已将 EV 鉴定为血栓风险的潜在预测指标6,7。EV 的促凝活性取决于凝血因子的表达，主要是组织因子（TF）和磷脂酰丝氨酸（PS）。具有强促凝活性的 EV 显著增强凝血酶原酶复合物和凝血酶原复合物的催化效率，从而促进凝血酶介导的纤维蛋白原转化及局部血栓形成8。在多种疾病中已观察到 EV 水平升高及其与高凝状态之间的因果关系9。因此，标准化 EV 的检测并报告其促凝活性是当前重要的研究方向10。
迄今为止，可用于检测细胞外囊泡（EVs）促凝活性的商用试剂盒仅有少数几种。由某商业公司生产的MP-活性检测法（MP-Activity assay）和MP-TF检测法（MP-TF assay）是用于测定血浆中EV促凝活性的功能性检测方法11。这些检测方法采用类似于酶联免疫吸附测定（ELISA）的原理，用于检测EV表面的磷脂酰丝氨酸（PS）和组织因子（TF）。然而，这些试剂盒价格昂贵，仅限于少数高水平研究机构使用。其操作流程复杂且耗时，难以在临床环境中推广应用。此外，一种 commercially developed 促凝磷脂（PPL）检测法将不含PS的血浆与待测血浆混合，通过测定凝血时间来定量检测PS阳性EV的水平12。然而，这些检测方法主要关注EV上的PS和TF，忽略了循环EV可能参与的其他凝血通路12。

Determination of the Procoagulant Activity of Extracellular Vesicle (EV) Using EV-Activated Clotting Time (EV-ACT)

采用血浆凝固时间测定用的粘弹性方法分析仪，测定富含细胞外囊泡（EV）血浆的凝血酶激活时间。该仪器由四个主要部件组成：电子信号转换器、探针、检测槽和加热元件（图1A，B）。探针利用高频低幅振荡来检测血浆黏度的变化。日常质量控制主要包括空气质控，用于评估检测平台的稳定性并识别探针对任何物理干扰的影响。性能质控则通过标准黏度油进行测试，以确保探针检测到的阻力值处于设定范围内。

我们的主要研究重点是脑外伤患者中的凝血功能障碍。在本研究中，我们尝试利用细胞外囊泡活性凝血时间来监测凝血功能的变化。我们的团队专注于脑损伤相关研究，在脑损伤中存在一种称为高凝状态的严重情况，这种状态难以准确测量。我们发现，患者血液中存在一些被称为细胞外囊泡的微囊泡，包括凋亡小体、微囊泡和外泌体。它们在促凝血能力方面存在差异，因此我们必须找到一种方法来测定这种能力。通过该方法，我们可以快速准确地检测细胞外囊泡之间的差异，这一点非常重要。基于这一检测方法，我们可以开展许多工作。我们已确认，在TPI小鼠和临床患者中，尤其是在外周循环中，会发生一些栓塞，这在临床上具有非常、非常严重的意义。与其它技术相比，我们的方案因其操作简便、成本低廉且速度快，具有显著优势。它能够在床旁进行血液凝固检测，无需复杂的实验室处理。我们的研究已建立一种新方法，可检测外周血的高凝状态。该方法不仅可用于TPI，还可应用于子痫前期、肿瘤及血液系统疾病等其他疾病。未来，我们计划研究EV ACT在临床高凝状态筛查中的应用，高凝状态会增加血栓形成的风险。首先，在室温下以120 ×g离心人全血20分钟，以去除血细胞，然后将上半部分的上清液转移至新管中。接着，将富血小板血浆再次离心以去除血小板，并将上半部分的上清液转移至新管中。然后以13,000 G离心乏血小板血浆两分钟，去除细胞碎片。将上清液的上半部分转移至新管中，开启凝块分析仪以检测细胞外囊泡（EV）活化的凝血时间，并将仪器预热至37摄氏度。安装一次性探针和检测杯，然后启动仪器的质量控制程序。完成质控运行后，将样本信息输入系统。然后向检测杯中加入 200 微升富含细胞外囊泡（EV）的血浆，随后加入 20 毫摩尔、170 微升的氯化钙。点击开始按钮，盖上盖子，使探针检测样本电阻的变化。不合格的EV样本在EV门附近表现出一个明显且粒径略大的聚类。相比之下，合格的富含EV样本则显示大多数信号位于EV门内，形成单一主群。EV浓度增加会缩短EV活化凝血时间，而EV浓度降低则延长EV活化凝血时间。来自子痫前期、髋部骨折和肺癌患者的富含EV血浆样本，其EV活化凝血时间显著短于健康志愿者。
