Currently patients are rarely afflicted with only one disease. Most of them suffer from several conditions which share risk factors, etiology, and most of the time are exacerbating course of one another. A phenotype specific for a certain condition is often shared with many other. We describe cases of three patients with obstructive sleep apnea (OSA), atrial fibrillation (AF) and erectile dysfunction (ED). These three conditions are highly prevalent in the general population. OSA is one of the most common types of sleep disordered breathing, affecting approximately 24% of men and 9% of women, at age of 30 to 60 years. AF is one of the most common arrhythmias, present in approximately 2% of population, and erectile dysfunction can be detected in 18% to 40% of general men population at age >20 years. Presence of those three conditions in one patient may not only be a coexistence but rather a new clinical syndrome. We present data which compels to consider OSA, AF, and ED as one clinical syndrome - OSAFED (Obstructive Sleep Apnea, Atrial Fibrillation and Erectile Dysfunction) which may have bigger, additive impact on cardiovascular risk profile than those three conditions alone. Introducing the OSAFED acronym into everyday clinical practice, would bring one tremendous advantage - remind physicians to screen every patient with diagnosed one the following OSA, AF or ED for the remaining two conditions, which would benefit in early diagnosis, and stop the vicious circle of mutual disease exacerbation.

Patients with autism spectrum disorders and intellectual disability can be clinically complex and often have limited access to psychiatric care. Because little is known about post-graduate clinical education in autism spectrum disorder and intellectual disability, we surveyed training directors of child and adolescent psychiatry fellowship programs. On average, child and adolescent psychiatry directors reported lectures of 3 and 4 h per year in autism spectrum disorder and intellectual disability, respectively. Training directors commonly reported that trainees see 1-5 patients with autism spectrum disorder or intellectual disability per year for outpatient pharmacological management and inpatient treatment. Overall, 43% of directors endorsed the need for additional resources for training in autism spectrum disorder and intellectual disability, which, coupled with low didactic and clinical exposure, suggests that current training is inadequate.

Training of child and adolescent psychiatry fellows in autism and intellectual disability.

Juvenile nasopharyngeal angiofibroma is a benign lesion with locally aggressive nature. Knowledge of its typical growth patterns is crucial for precise preoperative staging and adequate preoperative patient counseling. This pictorial essay focuses on characteristic radiological features and paths of invasive growth of this rare tumor. Also, the impact of accurate preoperative evaluation of tumor extensions on surgical planning and results of treatment are discussed.

Invasive growth patterns of juvenile nasopharyngeal angiofibroma: radiological imaging and clinical implications.

Objectives. We assessed suicide rates up to 6 months following discharge from US Department of Veterans Affairs (VA) nursing homes. Methods. In VA Minimum Data Set (MDS) records, we identified 281 066 live discharges from the 137 VA nursing homes during fiscal years 2002 to 2008. We used MDS and administrative data to assess resident age, gender, behaviors, pain, and indications of psychoses, bipolar disorder, dementia, and depression. We identified vital status and suicide mortality within 6 months of discharge through National Death Index searches. Results. Suicide rates within 6 months of discharge were 88.0 per 100 000 person-years for men and 89.4 overall. Standardized mortality ratios relative to age- and gender-matched individuals in the VA patient population were 2.3 for men (95% confidence interval [CI] = 1.9, 2.8) and 2.4 overall (95% CI = 2.0, 2.9). In multivariable proportional hazards regression analyses, resident characteristics, diagnoses, behaviors, and pain were not significantly associated with suicide risk. Conclusions. Suicide risk was elevated following nursing home discharge. This underscores the importance of ongoing VA efforts to enhance discharge planning and timely postdischarge follow-up. Suicide Mortality Following Nursing Home Discharge in the Department of Veterans Affairs Health System.

Cardiovascular disease are the leading cause of death worldwide. One of the most important diseases in this group is myocardial infarction. According to the Universal Definition developed by the European Society of Cardiology (ESC) myocardial infarction is divided into five main types basing on its cause. Type 2 myocardial infarction is secondary to ischemia due to either increased demand or decreased supply of oxygen (in example due to coronary artery spasm, anaemia, arrhythmia, coronary embolism, hypertension, or hypotension).

Clinical characteristics, etiology and occurrence of type 2 acute myocardial infarction.

Bacterial resistance is a major problem in the modern world, stemming in part from the build-up of antibiotics in the environment. Novel molecular approaches that enable an externally triggered increase in antibiotic activity with high spatiotemporal resolution and auto-inactivation are highly desirable. Here we report a responsive, broad-spectrum, antibacterial agent that can be temporally activated with light, whereupon it auto-inactivates on the scale of hours. The use of such a 'smart' antibiotic might prevent the build-up of active antimicrobial material in the environment. Reversible optical control over active drug concentration enables us to obtain pharmacodynamic information. Precisely localized control of activity is achieved, allowing the growth of bacteria to be confined to defined patterns, which has potential for the development of treatments that avoid interference with the endogenous microbial population in other parts of the organism.
