Complete blood vessel occlusion is required for the treatment of age-related macular degeneration (AMD). AMD is the leading cause of blindness in developed countries and current treatment regimes have potential to cause collateral damage, or do not remove pre-existing unwanted vasculature. It has been proposed that two-photon excitation (TPE) photodynamic therapy (PDT) can be applied to cause local blood vessel occlusion without damaging surrounding retinal tissues. The in ovo chicken chorioallantoic membrane (CAM) is used as the model for vascularization in the wet form of AMD; novel techniques for the utilization of the CAM are reported. Complete occlusion of CAM vessels approximately 15 microm in diameter is achieved using the clinically approved photosensitizer Verteporfin (Visudyne, QLT, Incorporated, Vancouver, British Columbia, Canada) and TPE activation. The average and peak irradiances used for treatment are 3.3x10(6) Wcm(2) and 3.7x10(11) Wcm(2), respectively. A total fluence of 1.1x10(8) Jcm(2) is the dosage required for successful occlusion, and it is expected that for optimal conditions it will be much less. These results are the first proof-of-principle evidence in the literature that indicate TPE-PDT can be used to occlude small blood vessels. Further investigation will help determine the utility of TPE-PDT for treating wet AMD, perhaps through targeting feeder vessels.

Highly localized reflectance measurements can be used to directly quantify scatter changes in tissues. We present a microsampling approach that is used to raster scan tumors to extract parameters believed to be related to the tissue ultrastructure. A confocal reflectance imager was developed to examine scatter changes across pathologically distinct regions within tumor tissues. Tissue sections from two murine tumors, AsPC-1 pancreas tumor and the Mat-LyLu Dunning prostate tumor, were imaged. After imaging, histopathology-guided region-of-interest studies of the images allowed analysis of the variations in scattering resulting from differences in tissue ultra-structure. On average, the median scatter power of tumor cells with high proliferation index (HPI) was about 26% less compared to tumor cells with low proliferation index (LPI). Necrosis exhibited the lowest scatter power signature across all the tissue types considered, with about 55% lower median scatter power than LPI tumor cells. Additionally, the level and maturity of the tumor's fibroplastic response was found to influence the scatter signal. This approach to scatter visualization of tissue ultrastructure in situ could provide a unique tool for guiding surgical resection, but this kind of interpretation into what the signal means relative to the pathology is required before proceeding to clinical studies.

An automated algorithm and methodology is presented to identify tumor-tissue morphologies based on broadband scatter data measured by raster scan imaging of the samples. A quasi-confocal reflectance imaging system was used to directly measure the tissue scatter reflectance in situ, and the spectrum was used to identify the scattering power, amplitude, and total wavelength-integrated intensity. Pancreatic tumor and normal samples were characterized using the instrument, and subtle changes in the scatter signal were encountered within regions of each sample. Discrimination between normal versus tumor tissue was readily performed using a K-nearest neighbor classifier algorithm. A similar approach worked for regions of tumor morphology when statistical preprocessing of the scattering parameters was included to create additional data features. This type of automated interpretation methodology can provide a tool for guiding surgical resection in areas where microscopy imaging cannot be realized efficiently by the surgeon. In addition, the results indicate important design changes for future systems.

PURPOSE: Cellular receptor targeted imaging agents present the potential to target extracellular molecular expression in cancerous lesions; however, the image contrast in vivo does not reflect the magnitude of overexpression expected from in vitro data. Here, the in vivo delivery and binding kinetics of epidermal growth factor receptor (EGFR) was determined for normal pancreas and AsPC-1 orthotopic pancreatic tumors known to overexpress EGFR. PROCEDURES: EGFR in orthotopic xenograft AsPC-1 tumors was targeted with epidermal growth factor (EGF) conjugated with IRDye800CW. The transfer rate constants (k (e,) K (12), k (21), k (23), and k (32)) associated with a three-compartment model describing the vascular delivery, leakage rate and binding of targeted agents were determined experimentally. The plasma excretion rate, k (e), was determined from extracted blood plasma samples. K (12), k (21), and k (32) were determined from ex vivo tissue washing studies at time points â‰¥24Â h. The measured in vivo uptake of IRDye800CW-EGF and a non-targeted tracer dye, IRDye700DX-carboxylate, injected simultaneously was used to determined k (23). RESULTS: The vascular exchange of IRDye800CW-EGF in the orthotopic tumor (K (12) and k (21)) was higher than in the AsPC-1 tumor as compared to normal pancreas, suggesting that more targeted agent can be taken up in tumor tissue. However, the cellular associated (binding) rate constant (k (23)) was slightly lower for AsPC-1 pancreatic tumor (4.1â€‰Ã—â€‰10(-4)Â s(-1)) than the normal pancreas (5.5â€‰Ã—â€‰10(-4)Â s(-1)), implying that less binding is occurring. CONCLUSIONS: Higher vascular delivery but low cellular association in the AsPC-1 tumor compared to the normal pancreas may be indicative of low receptor density due to low cellular content. This attribute of the AsPC-1 tumor may indicate one contributing cause of the difficulty in treating pancreatic tumors with cellular targeted agents.

High Vascular Delivery of EGF, but Low Receptor Binding Rate Is Observed in AsPC-1 Tumors as Compared to Normal Pancreas.

The sensitivity and specificity of in vivo magnetic resonance (MR) imaging is compared with production of protoporphyrin IX (PpIX), determined ex vivo, in a diffusely infiltrating glioma. A human glioma transfected with green fluorescent protein, displaying diffuse, infiltrative growth, was implanted intracranially in athymic nude mice. Image contrast from corresponding regions of interest (ROIs) in in vivo MR and ex vivo fluorescence images was quantified. It was found that all tumor groups had statistically significant PpIX fluorescence contrast and that PpIX contrast demonstrated the best predictive power for tumor presence. Contrast from gadolinium enhanced T1-weighted (T1W+Gd) and absolute T2 images positively predicted the presence of a tumor, confirmed by the GFP positive (GFP+) and hematoxylin and eosin positive (H&E+) ROIs. However, only the absolute T2 images had predictive power from controls in ROIs that were GFP+ but H&E negative. Additionally, PpIX fluorescence and T1W+Gd image contrast were linearly correlated in both the GFP+ (r = 0.79, p<1×10(-8)) and H&E+ (r = 0.74, p<0.003) ROIs. The trace diffusion images did not have predictive power or significance from controls. This study indicates that gadolinium contrast enhanced MR images can predict the presence of diffuse tumors, but PpIX fluorescence is a better predictor regardless of tumor vascularity.

PURPOSE: Receptor availability represents a key component of current cancer management. However, no approaches have been adopted to do this clinically, and the current standard of care is invasive tissue biopsy. A dual-reporter methodology capable of quantifying available receptor binding potential of tumors in vivo within a clinically relevant time scale is presented. PROCEDURES: To test the methodology, a fluorescence imaging-based adaptation was validated against ex vivo and in vitro measures of epidermal growth factor receptor (EGFR) binding potential in four tumor lines in mice, each line expected to express a different level of EGFR. RESULTS: A strong correlation was observed between in vivo and ex vivo measures of binding potential for all tumor lines (râ€‰=â€‰0.99, pâ€‰<â€‰0.01, slopeâ€‰=â€‰1.80â€‰Â±â€‰0.48, and interceptâ€‰=â€‰-0.58â€‰Â±â€‰0.84) and between in vivo and in vitro for the three lines expressing the least amount of EGFR (râ€‰=â€‰0.99, pâ€‰<â€‰0.01, slopeâ€‰=â€‰0.64â€‰Â±â€‰0.32, and interceptâ€‰=â€‰0.47â€‰Â±â€‰0.51). CONCLUSIONS: By providing a fast and robust measure of receptor density in tumors, the presented methodology has powerful implications for improving choices in cancer intervention, evaluation, and monitoring, and can be scaled to the clinic with an imaging modality like SPECT.

PROP&#xD3;SITO: La disponibilidad del receptor representa un componente clave del tratamiento del c&#xE1;ncer actual. Sin embargo, no se han adoptado enfoques para hacer esto cl&#xED;nicamente, y el actual est&#xE1;ndar de tratamiento es invasivo biopsia de tejido. Un reportero de la metodolog&#xED;a de doble capaz de cuantificar el potencial de receptor dispone de la uni&#xF3;n de los tumores in vivo dentro de una escala de tiempo cl&#xED;nicamente relevante se presenta. PROCEDIMIENTOS: Para probar la metodolog&#xED;a, una adaptaci&#xF3;n basada en im&#xE1;genes de fluorescencia ha sido validada contra la ex vivo e in vitro de las medidas del receptor de factor de crecimiento epid&#xE9;rmico (EGFR) potencial de uni&#xF3;n en cuatro l&#xED;neas tumorales en ratones, cada l&#xED;nea de espera para expresar un nivel diferente del EGFR. RESULTADOS: Un fuerte correlaci&#xF3;n se observ&#xF3; entre tanto in vivo como ex vivo de las medidas de potencial de uni&#xF3;n para todas las l&#xED;neas tumorales (r = 0,99, p &#x26;lt;0,01, pendiente = 1,80 &#xB1; 0,48, y la intersecci&#xF3;n = -0,58 &#xB1; 0,84) y entre in vivo e in in vitro para las tres l&#xED;neas que expresan la menor cantidad de receptores EGFR (r = 0,99, p &#x26;lt;0,01, pendiente = 0,64 &#xB1; 0,32, y de intercepci&#xF3;n = 0,47 &#xB1; 0,51). CONCLUSIONES: Al proporcionar una medida r&#xE1;pida y robusta de la densidad de los receptores en los tumores, la metodolog&#xED;a presentada tiene implicaciones de gran alcance para mejorar las opciones de intervenci&#xF3;n en el c&#xE1;ncer, la evaluaci&#xF3;n y monitoreo, y se puede escalar a la cl&#xED;nica con una t&#xE9;cnica de imagen como el SPECT.
