Biplane videoradiography kan kvantifiera axelkinematik med hög grad av noggrannhet. Axeln är ett av människokroppens mest komplexa ledsystem, med rörelse som sker genom samordnade åtgärder av fyra enskilda leder, flera ligament och cirka 20 muskler. Tyvärr är axelpatologier (t.ex. rotatormanschett tårar, ledförskjutningar, artrit) vanliga, vilket resulterar i betydande smärta, funktionshinder och minskad livskvalitet. Den specifika etiologin för många av dessa patologiskt tillstånd är inte helt klarlagd, men det är allmänt accepterat att axel patologi ofta är associerad med förändrad gemensamma rörelse. Tyvärr är det inte trivialt att mäta axelrörelser med den nödvändiga noggrannhetsnivån för att undersöka rörelsebaserade hypoteser. Radiografibaserade rörelsemätningstekniker har dock gett det framsteg som krävs för att undersöka rörelsebaserade hypoteser och ge en mekanistisk förståelse av axelfunktionen. Således är syftet med denna artikel att beskriva metoderna för att mäta axelrörelse med hjälp av ett anpassat biplanar videoradiography system. De specifika målen med denna artikel är att beskriva protokollen för att förvärva biplanar videoradiographic bilder av axelkomplexet, förvärva CT skanningar, utveckla 3D ben modeller, lokalisera anatomiska landmärken, spåra positionen och orienteringen av överarmsbenet, scapula och torso från biplanar radiografi bilder och beräkna de kinematiska resultatmåtten. Dessutom kommer artikeln att beskriva speciella överväganden som är unika för axeln när man mäter ledkinematik med detta tillvägagångssätt.
Axeln är ett av människokroppens mest komplexa ledsystem, med rörelse som sker genom samordnade åtgärder av fyra enskilda leder, flera ligament och cirka 20 muskler. Axeln har också det största rörelseomfånget av kroppens större leder och beskrivs ofta som en kompromiss mellan rörlighet och stabilitet. Tyvärr är axelpatologier vanliga, vilket resulterar i betydande smärta, funktionshinder och minskad livskvalitet. Till exempel påverkar rotatormanschett tårar cirka 40% av befolkningen över 601,2,3, med cirka 250 000 rotatormanschettreparationer som utförs årligen4, och en uppskattad ekonomisk börda på $ 3-5 miljarder per år i USA5. Dessutom är axelförskjutningar vanliga och är ofta förknippade med kronisk dysfunktion6. Slutligen är glenohumeral gemensamma artros (OA) ett annat betydande kliniskt problem som involverar axeln, med befolkningsstudier som visar att ungefär 15-20% av vuxna över 65 år har radiografi bevis på glenohumeral OA7,8. Dessa tillstånd är smärtsamma, försämrar aktivitetsnivåer och minskar livskvaliteten.
Även om patogeneserna i dessa villkor inte är helt förstådda, är det allmänt accepterat att förändrad axelrörelse är associerad med många axelpatologier9,10,11. Specifikt kan onormal gemensam rörelse bidra till patologi9,12, eller att patologin kan leda till onormal gemensamma rörelse13,14. Samband mellan gemensamma rörelse och patologi är sannolikt komplexa, och subtila förändringar i gemensamma rörelse kan vara viktiga i axeln. Till exempel, även om vinkel rörelse är den dominerande rörelsen som förekommer vid glenohumeral gemensamma, gemensamma översättningar förekommer också under bog rörelse. Under normala förhållanden överstiger dessa översättningar sannolikt inte flera millimeter15,16,17,18,19 och kan därför vara lägre än nivån på in vivo-noggrannheten för vissa mättekniker. Även om det kan vara frestande att anta att små avvikelser i ledrörelse kan ha liten klinisk inverkan, är det viktigt att också erkänna att den kumulativa effekten av subtila avvikelser över år av axelaktivitet kan överstiga individens tröskel för vävnadsläkning och reparation. Dessutom är in vivo-styrkorna vid glenohumeralleden inte oviktiga. Med hjälp av anpassade instrumenterade glenohumerala ledimplantat har tidigare studier visat att en höjning av en 2 kg vikt till huvudhöjd med en utsträckt arm kan resultera i glenohumerala ledkrafter som kan variera från 70% till 238% av kroppsvikten20,21,22. Följaktligen kan kombinationen av subtila förändringar i gemensamma rörelse och höga krafter koncentrerade över glenoidens små bärande yta bidra till utvecklingen av degenerativa bog patologier.
Historiskt sett har mätningen av axelrörelser genomförts genom en mängd olika experimentella metoder. Dessa metoder har inkluderat användning av komplexa kadaveriska testsystem utformade för att simulera axelrörelse23,24,25,26,27, videobaserade rörelseinspelningssystem med ytmarkörer28,29,31, ytmonterade elektromagnetiska sensorer32,33,34,35 , benstift med reflekterande markörer eller andra sensorer monterade36,37,38, statisk tvådimensionell medicinsk avbildning (dvs. fluoroskopi39,40,41 och röntgenundersökningar17,42,43,44,45), statisk tredimensionell (3D) medicinsk med hjälp av MR46,47, datortomografi48 och dynamisk, 3D enplan fluoroskopisk avbildning49,50,51. På senare tid har bärbara sensorer (t.ex. inertiella mätenheter) blivit populära för att mäta axelrörelser utanför laboratoriemiljön och i fria förhållanden52,53,54,55,56,57.
Under de senaste åren har det skett en spridning av biplan radiografi eller fluoroskopiska system utformade för att noggrant mäta dynamiska, 3D in vivo rörelser av axeln58,59,60,61,62. Syftet med denna artikel är att beskriva författarnas tillvägagångssätt för att mäta axelrörelse med hjälp av ett anpassat biplanar videoradiography system. Tekniken som beskrivs här övervinner flera nackdelar i samband med konventionella tekniker för bedömning av axelrörelse (dvs. kadaveriska simuleringar, 2D-avbildning, statisk 3D-avbildning, videobaserade motion capture-system, bärbara sensorer etc.) genom att tillhandahålla exakta mått på 3D-ledrörelse under dynamiska aktiviteter.
