Het doel van dit artikel is om beeld begeleiding te bieden voor minimaal invasieve transforaminale intercorporele fusie Fusion.
Transforaminale lumbale intercorporele fusie (TLIF) wordt vaak gebruikt voor de behandeling van Spinale stenose, degeneratieve schijf ziekte, en spondylolisthesis. Op deze techniek zijn minimaal invasieve operaties (MIS) toegepast met een geassocieerde afname van het geschatte bloedverlies (EBL), de duur van het verblijf in het ziekenhuis en de besmettings percentages, terwijl de resultaten met traditionele open chirurgie behouden blijven. Eerdere MIS TLIF-technieken omvatten belangrijke fluoroscopie die het personeel van de patiënt, chirurg en operatiekamer onderwerpt aan niet-triviale niveaus van stralingsblootstelling, met name voor complexe multi-level procedures. We presenteren een techniek die gebruik maakt van een intraoperatieve computertomografie (CT) scan om te helpen bij de plaatsing van de steel schroeven, gevolgd door traditionele fluoroscopie voor bevestiging van de plaatsing van de kooi. De patiënten worden op de standaard wijze gepositioneerd en er wordt een referentie boog Geplaatst in de posterieure superieure iliacale wervelkolom (PSIS) gevolgd door een intraoperatieve CT-scan. Dit maakt het mogelijk om de plaatsing van de steel op de beeld geleiding op te schroeven door middel van een ééninch huid incisie aan elke kant. In tegenstelling tot traditionele MIS-TLIF die in deze fase aanzienlijke fluoroscopische beeldvorming vereist, kan de operatie nu uitgevoerd worden zonder extra stralingsblootstelling aan de patiënt of de werkkamer personeel. Na voltooiing van de facetectomie en discectomie wordt de uiteindelijke plaatsing van de TLIF-kooi bevestigd met fluoroscopie. Deze techniek heeft de potentie om de operationele tijd te verminderen en de totale stralingsblootstelling te minimaliseren.
De tlif is een van de verschillende opties beschikbaar bij het overwegen van intercorporele fusie Fusion voor degeneratieve schijf ziekte en spondylolisthesis. De TLIF-techniek werd aanvankelijk ontwikkeld als reactie op complicaties in verband met de traditionalere posterieure lumbale intercorporele fusie (PLIF) benadering. Meer in het bijzonder, de TLIF geminimaliseerd terugtrekking van neurale elementen, waardoor het risico van zenuwwortel letsel evenals het risico van durale tranen, die kan leiden tot aanhoudende cerebrospinale vloeistof lekkage. Als eenzijdige benadering biedt de TLIF-techniek ook een beter behoud van de normale anatomie van de posterieure elementen1. De tlif kan ofwel open (O-tlif) of minimaal invasief (mis-tlif) worden uitgevoerd, en mis-tlif heeft bewezen een veelzijdige en populaire behandeling te zijn voor lumbale degeneratieve ziekte en spondylolisthesis2,3,4. Vergeleken met de O-TLIF is de MIS-TLIF geassocieerd met afgenomen bloedverlies, kortere ziekenhuisverblijf en minder verdovend gebruik; patiënt gerapporteerde en radiografische uitkomst maatregelen zijn ook vergelijkbaar tussen open-en mis-benaderingen, wat suggereert dat de mis-tlif een even effectieve maar mogelijk minder morbide procedure5,6,7, 8,9,10,11.
Een frequente beperking van de traditionele MIS-techniek is echter de zware afhankelijkheid van fluoroscopie die de patiënt, chirurg en operatiekamer medewerkers blootstelt aan niet-triviale stralingsdoses en fluoroscopie-tijd variërend van 46-147 s12. Meer recentelijk is echter het gebruik van intraoperatieve CT-geleide navigatie bestudeerd, met verschillende beschikbare systemen en beschreven in de literatuur, waaronder de O-arm/STEALTH, airo Mobile en Stryker spinale navigatiesystemen. 13 , 14 dit type navigeerde techniek heeft aangetoond dat het resulteert in nauwkeurige schroef plaatsing van de steel, terwijl ook het stralings risico voor de chirurg wordt geminimaliseerd15,16,17,18, 19. in dit artikel presenteren we een nieuwe techniek voor mis-TLIF die gebruik maakt van Image-guidance-gebaseerde pedile schroef plaatsing gevolgd door kooi en Rod plaatsing met traditionele fluoroscopie. Deze strategie heeft de potentie om de snelheid en nauwkeurigheid van de schroef plaatsing van de pedile te verhogen, terwijl de blootstelling aan straling wordt geminimaliseerd door het personeel van de patiënt en de operatiekamer.
Er zijn verschillende kritieke stappen voor de beschreven procedure. De eerste kritieke stap is het registratieproces. De referentie boog moet worden geplaatst in massief bot en moet op de juiste wijze worden georiënteerd om te voorkomen dat de schroef plaatsing van de S1-Steel indien nodig wordt verstoord. De tweede kritieke stap is het handhaven van de nauwkeurigheid van de navigatie nadat een intraoperatieve CT-scan wordt uitgevoerd, die kan worden gedaan door het identificeren van normale anatomische structuren en h…
