Resting-state functional-connectivity MRI has identified abnormalities in patients with a wide range of neuropsychiatric disorders, including epilepsy due to malformations of cortical development. Transcranial Magnetic Stimulation in combination with EEG can demonstrate that patients with epilepsy have cortical hyperexcitability in regions with abnormal connectivity.
Resting-state functional connectivity MRI (rs-fcMRI) is a technique that identifies connectivity between different brain regions based on correlations over time in the blood-oxygenation level dependent signal. rs-fcMRI has been applied extensively to identify abnormalities in brain connectivity in different neurologic and psychiatric diseases. However, the relationship among rs-fcMRI connectivity abnormalities, brain electrophysiology and disease state is unknown, in part because the causal significance of alterations in functional connectivity in disease pathophysiology has not been established. Transcranial Magnetic Stimulation (TMS) is a technique that uses electromagnetic induction to noninvasively produce focal changes in cortical activity. When combined with electroencephalography (EEG), TMS can be used to assess the brain’s response to external perturbations. Here we provide a protocol for combining rs-fcMRI, TMS and EEG to assess the physiologic significance of alterations in functional connectivity in patients with neuropsychiatric disease. We provide representative results from a previously published study in which rs-fcMRI was used to identify regions with abnormal connectivity in patients with epilepsy due to a malformation of cortical development, periventricular nodular heterotopia (PNH). Stimulation in patients with epilepsy resulted in abnormal TMS-evoked EEG activity relative to stimulation of the same sites in matched healthy control patients, with an abnormal increase in the late component of the TMS-evoked potential, consistent with cortical hyperexcitability. This abnormality was specific to regions with abnormal resting-state functional connectivity. Electrical source analysis in a subject with previously recorded seizures demonstrated that the origin of the abnormal TMS-evoked activity co-localized with the seizure-onset zone, suggesting the presence of an epileptogenic circuit. These results demonstrate how rs-fcMRI, TMS and EEG can be utilized together to identify and understand the physiological significance of abnormal brain connectivity in human diseases.
Transcraniële magnetische stimulatie (TMS) is een middel om niet-invasief stimuleren gebieden van cortex via elektromagnetische inductie. In TMS is een groot, maar ruimtelijk beperkt magnetische flux gebruikt om een elektrisch veld te induceren in een doel corticale gebied en daardoor de activiteit van de onderliggende zenuwweefsel moduleren. TMS-EEG maatregelen belangrijke afwijkingen aangetoond bij ziekten variërend van schizofrenie 17 tot 18 ADHD en bij aandoeningen van het bewustzijn zoals persistente vegetatieve toestand 19. Bovendien hebben verschillende groepen geïdentificeerd EEG correlaten van de gepaarde-pulse TMS-EMG metrics dat abnormale bij patiënten met epilepsie 20,21 zijn. Van bijzonder belang, hebben eerdere studies ook gesuggereerd dat abnormale stimulatie opgewekte EEG-activiteit is waargenomen bij patiënten met epilepsie 22-25.
Een ander middel voor het evalueren van de hersenen circuits is via resting state functionele connectiviteit MRI (rs-fcMRI), een techniek die de correlaties evalueert de loop der tijd in het bloed zuurstof niveau afhankelijk (BOLD) signaal van verschillende hersengebieden 26. studies die gebruikrs-fcMRI hebben aangetoond dat de menselijke hersenen is georganiseerd in verschillende netwerken van interagerende gebieden 26-29, die neuropsychiatrische ziekten kunnen optreden binnen specifieke grootschalige gedistribueerde neurale netwerken die door rs-fcMRI 30, en dat de hersennetwerken die via RS- fcMRI zijn vaak abnormaal in neuropsychiatrische ziektebeelden 31,32. In termen van potentiële klinische toepassingen, RS-fcMRI heeft een aantal voordelen ten opzichte van conventionele taakgericht fMRI applicatie 33, met inbegrip van minder afhankelijkheid van onderwerp samenwerking en de bezorgdheid over de variabele prestaties. Bijgevolg is er onlangs een explosie van studies waarin rs-fcMRI veranderingen in verschillende ziektebeelden geweest. Een van de beperkingen uit RS-fcMRI is de moeilijkheid bij het bepalen of en hoe correlaties (of anticorrelations) in het BOLD signaal betreffen de elektrofysiologische interacties die basis van neuronale communicatie vormen. Een gerelateerd probleem is dat het often onduidelijk of de RS-fcMRI veranderingen zien in verschillende ziekte staten hebben fysiologische betekenis. In het bijzonder met betrekking tot epilepsie, is het onduidelijk of afwijkingen in rs-fcMRI zijn uitsluitend te wijten aan interictale epileptiforme transiënten, of onafhankelijk bestaan van dergelijke elektrofysiologische afwijkingen; simultane EEG-fMRI is nodig om te helpen bij het evalueren tussen deze mogelijkheden 34.
Zoals TMS kan worden gebruikt om tijdelijke of langdurige veranderingen in de activeringen van verschillende corticale gebieden te produceren, TMS studies verschaffen een middel causaal beoordeling van de significantie van verschillende rusttoestand fMRI connectiviteitspatronen. Een andere benadering zou betekenen met behulp van TMS-EEG causaal beoordelen van de fysiologische betekenis van de verschillende rust-state fcMRI patronen. In het bijzonder, kan men het testen van de hypothese dat de regio's die abnormale functionele connectiviteit in een bepaalde toestand ziekte vertonen een ander antwoord op stimulatie bij patiënten dan bij gezonde personen moeten tonen, en dat deze fysiologische afwijkingen aanwezig zijn specifiek (of vooral) met stimulatie van de abnormaal aangesloten regio.
Om het bovenstaande te illustreren, geven we een voorbeeld van een recent onderzoek waarin RS-fcMRI, TMS en EEG gecombineerd om corticale hyperexcitability bij patiënten met epilepsie onderzoeken vanwege de ontwikkeling hersenenabnormaliteit periventriculaire nodulaire heterotopia (PNH) 37. Patiënten met PNH huidige klinisch met adolescent- of adult-onset epilepsie, het lezen van handicap, en de normale intelligence, en hebben abnormale knobbeltjes van grijze stof grenzend aan de laterale ventrikels op neuroimaging 38,39. Eerdere studies hebben aangetoond dat deze periventriculaire nodules van heterotope grijze stof structureel en functioneel zijn verbonden met afzonderlijke foci in de neocortex 40,41, en epileptische aanvallen kunnen afkomstig zijn van neocortical gebieden, heterotope grijze stof, of beide tegelijkertijd 42, wat suggereert dat in epileptogenese deze patiënten is een circuit fenomeen. Via rusttoestand fc-MRI TMS-EEG gids, we aangetoond dat patiënten met actieve epilepsie door PNH hebben bewijs van corticale hyperexcitabiliteit en dat hyperexcitatie lijkt te worden beperkt tot gebieden met abnormale functionele connectiviteit naar de diepe knobbeltjes.
Het protocol wordt uitgevoerd in twee afzonderlijke sessies. Tijdens de eerste sessie, zijn bloed-oxygenatie structurele en resting state-niveau-afhankelijke (BOLD) contrast MRI sequenties verworven(Patiënten) of door structurele MRI sequenties (voor de gezonde controles). Tussen de eerste en de tweede sessie, rust-state functionele connectiviteit analyse gebruikt om de corticale doelen definiëren voor de patiënten en de MNI coördinaten voor deze doelen worden verkregen. De equivalente corticale doelen (gebaseerd op MNI coördinaten) worden vervolgens de voor elke gezonde controle onderworpen. In de tweede sessie, wordt de TMS-EEG data verkregen.
In de in dit document voorbeeld werden functionele connectiviteit MRI analyses uitgevoerd met een in-house software toolbox en MRI software 43,44. -Neuro genavigeerd TMS werd uitgevoerd met een transcraniële magnetische stimulator met real-time MRI neuronavigatie. EEG werd opgenomen met een 60-kanaals TMS-compatibel systeem, dat een bemonster- en houdschakeling aan versterker verzadiging te vermijden door TMS gebruikt. EEG-gegevens werden geanalyseerd met behulp van aangepaste scripts en de EEGLAB toolbox 45 (version 12.0.2.4b) loopt in MATLAB R2012b.
