EUCAST heeft een protocol voor directe antimicrobiële gevoeligheidstests (AST) ontwikkeld voor geautomatiseerde bloedculturen. De afhankelijkheid van op massaspectrometrie gebaseerde microbiële identificatie kan echter worden vermeden door gebruik te maken van een direct protocol voor de bereiding van inoculum in een geautomatiseerd microbieel identificatiesysteem. Deze aanpak kan AST-rapporten opleveren binnen 24 uur na monsterafname.
Gramnegatieve (GN) sepsis is een medisch noodgeval waarbij de behandeling in omgevingen met beperkte middelen berust op conventionele microbiologische kweektechnieken, wat binnen 3-4 dagen resultaten oplevert. Zowel EUCAST als CLSI erkennen deze vertraging in de doorlooptijd (TAT) en hebben protocollen ontwikkeld voor het bepalen van AST-resultaten rechtstreeks uit positief gemarkeerde geautomatiseerde bloedkweekflessen (+aBC’s). Het EUCAST rapid AST (RAST)-protocol werd voor het eerst geïntroduceerd in 2018, waarbij breekpunten in de zonediameter voor vier veel voorkomende etiologische agentia van GN-sepsis, namelijk Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa en Acinetobacter baumannii-complex , kunnen worden gerapporteerd. De klinische laboratoria die deze methode in hun routinematige workflow hebben geïmplementeerd, vertrouwen echter op microbiële identificatie op basis van massaspectrometrie, die niet gemakkelijk beschikbaar is, waardoor de implementatie ervan in omgevingen met beperkte middelen wordt uitgesloten. Om dit te omzeilen, evalueerden we een direct inoculumprotocol (DIP) met behulp van een commercieel geautomatiseerd microbieel identificatie- en antimicrobieel gevoeligheidstestsysteem (aMIAST) om vroege microbiële identificatie mogelijk te maken binnen 8 uur na positieve markering van aBC. We hebben dit protocol van januari tot oktober 2023 geëvalueerd om de vier RAST-rapporteerbare GN (RR-GN) in de positief gemarkeerde aBC te identificeren. De microbiële identificatieresultaten in DIP werden vergeleken met het standaard inoculumvoorbereidingsprotocol (SIP) in aMIAST. Van de 204 +aBC’s met monomorfe GN (+naBC) werd één van de 4 RR-GN geïdentificeerd in 105 +naBC’s door SIP (E. coli: 50, K. pneumoniae: 20, P. aeruginosa: 9 en A. baumannii complex: 26). Hiervan werd 94% (98/105) correct geïdentificeerd door DIP, terwijl de percentages grote fouten en zeer grote fouten respectievelijk 6% (7/105) en 1,7% (4/240) bedroegen. Wanneer DIP voor microbiële identificatie wordt uitgevoerd met behulp van de EUCAST RAST-methode, kunnen binnen 24 uur na ontvangst van het monster voorlopige klinische rapporten worden verstrekt. Deze aanpak heeft het potentieel om de TAT aanzienlijk te verminderen, waardoor een vroege instelling van geschikte antimicrobiële therapie mogelijk wordt.
Sepsis, een belangrijk wereldwijd gezondheidsprobleem, wordt gedefinieerd als levensbedreigende orgaandisfunctie als gevolg van een ontregelde reactie van de gastheer op infectie. De Global Burden of Diseases Study schatte dat er in 2017 wereldwijd 48,9 miljoen gevallen van sepsis en 11 miljoen sepsisgerelateerde sterfgevallen waren, wat goed was voor bijna 20% vanalle wereldwijde sterfgevallen. Ongeveer 2/3e van de bloedbaaninfecties (BSI) die sterfte veroorzaken, is te wijten aan gramnegatieve bacteriële pathogenen2. De belangrijkste doodsoorzaken onder gramnegatieven (GN) zijn Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa en Acinetobacter baumannii, die ongeveer 40% van de gevallen uitmaken onder 33 bacteriëlepathogenen.
Bloedculturen blijven de gouden standaard voor het diagnosticeren van BSI, en snelle microbiële identificatie samen met de resultaten van antimicrobiële gevoeligheidstests (AST) is de sleutel tot behandeling. Er wordt geschat dat de kans op sterfte met 9% toeneemt met elke uur vertraging bij het instellen van geschikte antimicrobiële stoffen bij sepsis3. De doorlooptijd (TAT) van microbiologisch positieve bloedkweekrapporten met AST-resultaten is ongeveer 48-72 uur met de beschikbare microbiologische hulpmiddelen in omgevingen met beperkte middelen, zelfs met geautomatiseerde systemen. Als gevolg van deze ondermaatse TAT worden breedspectrum antimicrobiële stoffen empirisch gebruikt, wat bijdraagt aan het ontluikende probleem van antimicrobiële resistentie (AMR). EUCAST en CLSI erkennen deze dringende noodzaak om TAT voor microbiologische kweektechnieken voor sepsis te verminderen en evolueren naar het rechtstreeks uitvoeren van ASAT uit positief gemarkeerde bloedkweekflessen (+aBC)4,5.
In 2018 introduceerde EUCAST voor het eerst de snelle ASAT (RAST)-methode voor het bepalen van ASAT door middel van de Kirby-Bauer-schijfdiffusiemethode bij korte incubatietijden, d.w.z. 4 uur, 6 uur en 8 uur, rechtstreeks vanaf +aBC 6,7. De methode is momenteel gevalideerd voor het bepalen van ASAT voor +aBC’s die een van de 8 meest voorkomende oorzaken van BSI bevatten, namelijk E. coli, K. pneumoniae, P. aeruginosa en A. baumannii-complex onder gramnegatieven en Staphylococcus aureus, Enterococcus faecalis, E. faecium en Streptococcus pneumoniae onder grampositieven8. De breekpunten voor AST-bepaling met verschillende tijdsintervallen worden verstrekt volgens de hierboven genoemde microbiële soorten. Daarom is microbiële identificatie noodzakelijk voordat AST-resultaten categorisch worden geïnterpreteerd. De RAST-standaard specificeert echter niet de methode om microbiële identificatie binnen dit tijdsbestek mogelijk te maken.
In de meeste studies die de EUCAST RAST-methode in hun setting evalueerden, werd gebruik gemaakt van op massaspectrometrie gebaseerde microbiële identificatie na korte incubatie op vergulde media om micro-organismen te identificeren 9,10,11,12,13,14,15,16,17 . Massaspectrometrie-instrumenten zijn echter niet overal beschikbaar, vooral niet in lage- tot middeninkomenslanden (LMIC’s), wat het potentiële nut van deze methode sterk beperkt. Weinig studies hebben de implementatie van deze methode in hun centra gemeld zonder gebruik te maken van massaspectrometrie 18,19,20. Tayşi et al.18 rapporteerden een brede categorisering van GN onder Enterobacterales, Pseudomonas en Acinetobacter spp. op basis van gramkleuringsmorfologie en oxidasetest voordat de AST-resultaten werden geïnterpreteerd. In andere studies van dit centrum, door Gupta et al.19 en Siddiqui et al.20, werd microbiële identificatie op soortniveau gedaan door een bacteriepellet te bereiden uit het positief gemarkeerde bloedbouillonmengsel en deze te inoculeren op de conventionele biochemische tests. Hoewel Tayşi et al.18 geen commentaar gaven op de nauwkeurigheid van microbiële identificatie met hun benadering, rapporteerden Gupta et al.19 dat met hun benadering in 165/176 (94%) gevallen, een RAST-rapporteerbare gramnegatief (RR-GN), d.w.z. een van E. coli, K. pneumoniae, P. aeruginosa en A. baumannii complex. Met de laatste benadering werd het lezen van RAST-resultaten echter retrospectief gedaan met behulp van breekpunten met een zonediameter van 8 uur, pas na de volledige incubatie van conventionele biochemische resultaten, d.w.z. 18-24 uur na inoculatie, en de gemiddelde tijd voor rapportage was ongeveer 2 dagen.
Om de TAT van klinische rapporten verder te verminderen, stellen we een alternatieve methodologie voor om vroege identificatie van GN aanwezig in +aBC’s mogelijk te maken met behulp van aMIAST. Vóór de introductie van op massaspectrometrie gebaseerde microbiële identificatiesystemen, werden deze geautomatiseerde identificatiesystemen beschouwd als de zorgstandaard voor microbiële identificatie, waarbij identificatie mogelijk werd gemaakt door colorimetrische en/of fluorometrische veranderingen die werden geïnduceerd door testbacteriën wanneer ze werden geïnoculeerd in geminiaturiseerde biochemische tests die in een cassette werden bewaard en de resultaten matchten met hun isolaatdatabase. De gemiddelde tijd tot identificatie in deze systemen is ongeveer 4 uur tot 8 uur, maar ze worden beperkt door het feit dat de fabrikanten aanbevelen om microben ‘s nachts te laten groeien voordat hun respectieve identificatiekaarten kunnen worden geïnoculeerd. Deze vereiste beperkt hun bruikbaarheid bij het verkorten van de tijd om te rapporteren aanzienlijk.
Er zijn maar weinig studies die methoden hebben geëvalueerd om microben van +aBC’s direct te identificeren met behulp van deze geautomatiseerde systemen 21,22,23,24,25,26,27. In het geval van +aBC’s die monomorfe GN bevatten, toonden de meeste onderzoeken een uitstekende overeenstemming aan tussen directe inoculatie uit bacteriële pellets gemaakt van een positief bloedbouillonmengsel en standaard kolonie-incubatie. In het geval van grampositieven waren de concordantiepercentages echter suboptimaal. Aangezien de gemiddelde tijd tot positiviteit van +aBC’s tussen 8 uur en 16 uur ligt en de identificatie van GN ~4 uur tot 8 uur duurt in een geautomatiseerd microbieel identificatiesysteem, veronderstellen we dat door gebruik te maken van een direct inoculatieprotocol bij de geautomatiseerde microbiële identificatie, we de klinische rapportage van +aBC’s met GN met een RR-GN binnen 24 uur na monsterontvangst kunnen voltooien.
Setting voor de studie
De huidige studie werd van januari tot oktober 2023 uitgevoerd in het klinische bacteriologisch laboratorium van een academisch instituut voor tertiaire zorg van nationaal belang (INI) met 950 bedden in Centraal-India. Het laboratorium is uitgerust met een continu bloedkweek monitoring systeem (CBCMS) en aMIAST. Het bacteriologisch laboratorium is de klok rond functioneel met de beschikbaarheid van technici en microbiologen voor het verwerken en rapporteren van elk positief gemarkeerd bloedkweekflesje (+aBC’s).
Microbiële methoden die hier worden gebruikt
De workflow van het onderzoek is weergegeven in figuur 1. De +aBC’s die monomorfe GN’s (+naBC) vertonen, werden verwerkt door directe inoculatie van overeenkomstige identificatiekaarten om identificatie en AST mogelijk te maken met behulp van het EUCAST RAST-protocol. Deze resultaten werden vergeleken met de standaardbehandeling (SoC) methode voor +aBC’s, d.w.z. subcultuur op conventionele vergulde media door middel van schapenbloedagar (SBA), chocolade-agar (CA) en MacConkey-agar (MA), aëroob geïncubeerd gedurende 16 uur tot 24 uur, gevolgd door identificatie- en AST-kaarten gegeven door aMIAST wanneer geïsoleerde kolonies verschijnen. Bloedculturen die grampositieve kokken, grampositieve bacillen, ontluikende gistcellen en ≥2 verschillende micro-organismen op initiële gramkleuring of vergulde media vertoonden, werden uitgesloten van het onderzoek.
Met behulp van DIP zijn we erin geslaagd de RR-GN’s te identificeren met een aanzienlijke diagnostische nauwkeurigheid. De gemiddelde TTI na positieve markering van aBC was slechts 507 min (~ 8,5 uur). Wanneer het dus wordt gedaan in combinatie met de EUCAST RAST-methode voor AST-bepaling, kan het isolaatidentificatie geven bij een AST-leestijd van 8 uur. Deze aanpak heeft het potentieel om de EUCAST RAST-methode te implementeren, waardoor identificatie op basis van massaspectrometrie overbodig wordt. Dit is een zegen vo…
