Summary

Abstract
颧骨植入物(ZIs)是解决严重萎缩性无牙颌和上颌骨缺陷病例的理想方法,因为它们取代了广泛的骨增量并缩短了治疗周期。然而,放置ZI存在相关风险,例如穿透眶腔或颞下窝。此外,多个ZI的放置使这种手术有风险并且更难以执行。潜在的术中并发症非常危险,并可能导致无法弥补的损失。在这里,我们描述了一种实用,可行且可重复的实时手术导航系统方案,用于将四颧植入物精确放置在残余骨患者的严重萎缩上颌骨中,不符合常规植入物的要求。根据该协议,我们部门已有数百名患者接受了 ZI。临床结局令人满意,术中和术后并发症较低,输注设计图像和术后三维图像所指示的准确性较高。在整个手术过程中应使用此方法,以确保ZI放置安全。
Introduction
在 1990 年代,Branemark 引入了一种替代骨移植技术,即颧骨植入物 (ZI),也称为颧骨固定装置1。它最初用于治疗创伤受害者和上颌骨结构有缺陷的肿瘤切除患者。上颌骨切除术后,许多患者仅在颧骨体或颧骨的额叶延伸处保留锚定1,2,3。
最近,ZI技术已广泛用于上颌骨严重再吸收的无牙颌和齿状患者。ZI植入物的主要适应症是萎缩性上颌骨。对于具有丰富临床经验的外科医生来说,在即时负荷系统(固定修复学)中使用四个ZI是实用的,并且它似乎是骨移植技术的绝佳替代方法2,4。然而,徒手或使用手术模板进行指导放置 ZI 时存在风险。风险包括肺泡内放置不准确、眶腔或颞下窝穿透以及颧突内放置不当5.多个ZI的放置使这种手术有风险且难以进行。因此,提高ZI放置的精度对其临床使用和安全性至关重要。
实时手术导航系统提供了一种不同的方法。它通过分析术前和术中计算机断层扫描图像提供实时和完全可视化的轨迹。借助实时导航系统,通过复杂的手术和治疗提高了精度和安全性5,6。使用实时手术导航系统开发了一种实用、可行且可重复的方案,以精确地将 ZI 放置在严重萎缩的上颌骨5,7,8,9,10 中。通过该方案,我们已经治疗了数百名患者,临床结果令人满意5,6,7,8,9,10。在这里,我们提供了有关治疗程序的详细信息的协议。

Protocol
所有临床方案均由上海交通大学医学院附属第九人民医院医学伦理审查委员会批准(SH9H-2020-T29-3)。
1. 患者选择
- 患者纳入标准如下(表1)。
- 确保患者呈现完全无牙颌或部分无牙颌,牙齿很少松动(图 1A-G)。
- 确保患者上颌骨严重萎缩,骨体积不足,无法在前上颌骨和/或后上颌骨进行常规种植体放置。
- 确保患者年龄在 18-80 岁之间,没有全身性疾病。
- 确保患者已接受锥形束计算机断层扫描 (CBCT) 并分析了 DICOM 数据9。
注意:术前CBCT是使用具有以下扫描参数的商业仪器获得的:7.1 mA,96 kV,0.4 mm体素大小,视野为23 cm (D) x 26 cm (H),扫描时间为18 s。- 使用计划软件,确认前磨牙和磨牙区域的上颌后骨高度范围为1至3毫米(Cawood和Howell VI级)11 (表2)。
- 确保测量的前上颌骨宽度不足以放置直径至少为 3.75 毫米的常规植入物,而无需额外的骨移植或高度不足,即使采用标题为7、12、13 的方法,也不允许放置短于 10 毫米的植入物(图 1G1-G6)。
注意:放置ZI顶点的骨厚度应至少为5.75mm14 (图2A - B)(表1)。
- 患者排除标准如下(表1)。
- 足够的骨骼进行常规种植治疗。
- 狭窄的残余骨,颊骨移植被认为更合适。
- 未经治疗的上颌窦炎或上颌窦囊肿。
- 口腔手术和植入物放置的局部或全身禁忌症。
- 对于无牙颌患者,上颌残余骨量不符合Cawood Howell分类11的V级或VI级标准。
2. 迷你螺钉植入
- 进行局部麻醉以麻醉患者的上颌骨、双侧上颌骨粗隆、中线腭缝合线和鼻前脊柱两侧。
- 在局部麻醉下在剩余的上颌骨中植入 7 至 8 颗迷你螺钉(直径:1.0 mm,长度:9.0 mm,方腔:1.0 mm),作为双侧上颌骨结节、中线腭缝合线和鼻棘肌轨迹规划前的登记点。
- 选择双侧上颌骨粗隆、中线腭缝合线和鼻前脊柱两侧作为基准点的骨锚定区域(图 3A-C)。
注意: 为提高导航精度,迷你螺钉应均匀分散地放置在指定区域。
3. 术前全血细胞CT扫描以制定计划
- 使用以下扫描参数执行CBCT:7.1 mA,96 kV,0.4 mm体素大小,视野23 cm (D) x 26 cm (H),扫描时间为18 s。
4. 设置注册点
- 通过DVD驱动器将CBCT数据导入术前计划软件。
- 将所有微型螺钉标记为术中成像配准的配准点(图3D)。
- 在钛迷你螺钉的中心表面上标记点;这必须按一定的顺序设置。
注意:设置配准点后,参考Carlos Aparicio15提出的颧骨解剖引导方法,确保ZI的口内冠状入口点位于或附近。前ZI应位于外侧门牙/犬齿区域,后ZI应位于第二前磨牙/第一磨牙区域。内侧种植体的顶点应放置在远端种植体的顶点之上。根据先前的研究,后上区和颧骨中心是内侧植入物顶点和远端种植体顶点的理想位置16。长度只能在 30.0 至 52.5 毫米范围内选择。圆柱形机器人轨迹可以规划为钻孔路径(图3E-K)。
- 在钛迷你螺钉的中心表面上标记点;这必须按一定的顺序设置。
5. 计划四联ZI手术
注意:此协议需要导航系统。
6. 外科手术
- 全身麻醉后将患者以仰卧位躺在手术台上。
注意:最好在将患者置于全身麻醉之前将患者置于此位置。否则,很难切换位置。 - 固定头骨参考:使用单个 1.5 x 6 mm 的自攻钛螺钉将颅骨参考底座牢固地固定在颅骨上。将参考阵列固定到底座上,并用三个标记的反射球组装(图 4A-C)。将导航系统摄像头放在 1 点钟位置以监控头骨参考。
- 配准:使用带有定制反射球的定位探头,专门针对个体患者设置导航系统,以逐个接触微型螺钉的外表面。然后,在导航屏幕上显示可用的矢状面、冠状面、轴向和 3D 重建图像(图 4D-E)。
注意:注册程序完成后,检查每个基准标记的精度。如果误差主要为 <1.0 mm,则结果是可以接受的。否则,应重复注册过程,直到错误变得可接受。 - 标准化:在手术中使用钻孔之前对其进行标准化。使用带有不同直径孔的校准块来标准化钻头:直径 2.5 毫米(圆钻)、2.9 毫米(导向钻)和 3.5 毫米(延伸钻)。钻头应由外科医生直接连接到块的底部,然后助手需要将接口调整到校准模块中。该过程完成后,设备将发出声音。
- 牙龈瓣开口:在手术导航的引导下确定切口的范围。抬起全层皮瓣,以便有足够的视野暴露计划的植入部位。
注意:骨膜抬高的范围应包括牙槽嵴、上颌骨侧壁和颧骨下缘。 - 入口点标记:首先,借助导航探头找到入口点。然后,使用颧骨手机固定入口点。接下来,用探针找到颧骨的入口。使用颧骨手机准备颧骨的入口点(图4F-G)。
注意:确保操作员和助手都注意真实的手术区域,以防止导航系统出错。 - 初始准备:执行钻孔程序,确保它按计划遵循从入口到出口点的轨迹。首先使用 2.9 mm 钻头准备从使用导航探头定位的入口点到颧骨入口的路径。先准备内侧的,然后准备远端的。
注意:用导航探头检查每个步骤,以确认路径根据设计的术前计划正确(图4H-I)。 - 加宽植入床:使用手机将路径从颧骨入口延伸到颧骨表面设计的终点。
注意:请助手将手放在侧侧眶壁的表面上,以确保其安全。确保外科医生注意导航屏幕而不是手术区域。 - 读数和测量:使用直径为 3.5 mm 的膨胀钻头放大轨迹。使用测量条和导航探头检查轨迹的方向和位置。使用测量工具识别植入物的长度(图4B)。
注意:如果深度不符合计划长度的要求,最好将其准备到设定的深度。 - 植入:使用特定的手动工具植入 ZI。
- 缝合:ZI植入后,使用导航探头验证定位正确。在植入物上放置多单元基台和愈合帽,并用聚丙烯4-0缝合线缝合切口。去除参考系后,还应缝合发际线切口。
7. 术后用药
- 给患者服用 5 天的抗生素、镇痛药和漱口水溶液(0.12% 氯己定)。
8. 立即恢复
- 在72小时内对患者进行立即恢复(图5C-G)。
9. 图像集成
- 获得术后CBCT扫描图像和全景X光片,以在手术后72小时内评估ZI位置(图5A-B)。将术后数据导出到计划软件,以叠加术后CBCT和术前手术计划的图像,比较入口点,终点和角度偏差的位置(图5H-I,表4)。

Representative Results
入组患者是一名60岁的女性,没有任何系统性疾病(图1A-D,F)。CBCT扫描后,上颌前部的牙槽嵴小于2.9 mm,而上颌后区的残余骨高度小于2.4 mm(图1E,G和表1)。颧骨的宽度和厚度分别约为22.4-23.6毫米和6.1-8.0毫米(图2,表3)。根据颧骨解剖引导方法,前ZI的入口位于犬区的水平,后ZI位于第二前磨牙(图3E)。前ZI边缘与眼眶之间的距离在右侧为5.2 mm,在左侧为3.6 mm,而后ZI边缘与翼腭窝之间的距离在右侧为2.9 mm,在左侧为4.3 mm(图3F-K)。
手术是使用导航系统进行的(图4A-G)。手术后,患者在3天内接受了临时修复体,解决了美学和发音问题(图5C-G)。术后CBCT扫描及影像整合显示,从左后ZI到左前ZI的入口到左前ZI的误差,再到右前ZI和最后到右后ZI的误差分别为1.25 mm、1.35 mm、1.35 mm和1.85 mm。目标从左后ZI到右后ZI的误差分别为2.25 mm、1.55 mm、2.40 mm和1.20 mm。ZI角的误差分别为3.50°、3.59°、3.20°和2.15°(图5H-I,表4)。

Discussion
使用移植物对萎缩性上颌骨进行重建是困难的,因为它需要良好的手术技术,覆盖移植物上的高质量软组织,大量的患者合作以及有利于顶端修复的患者17,18。在上颌萎缩患者中放置用于重建的牙科植入物是一项重大的临床挑战。面部骨吸收的模式与年龄有关,在无牙颌上颌中尤其明显,在使用完全可拆卸假体的患者中尤其明显19,20。因此,ZI的发展代表了涉及肿瘤,创伤和外胚层发育不良病例的有效替代方案。该技术的主要优点是只需要一种手术方法,从而减少了治疗阶段的数量并实现了立即恢复的目标。立即加载程序还可以提高患者的美观和功能满意度,因为在这种技术中,没有无牙期。植入后,立即进行修复21.它还避免了在供体部位进行进一步的骨采集手术的需要22,23。通过在前部区域添加两到四个标准植入物或通过四重ZI方法24,25,它在上颌后区域的颧骨中实现了稳定的骨锚定,其骨质量为IV型,不允许插入标准植入物。目前,这种手术技术的适应症已应用于创伤,严重牙周炎和外胚层发育不良的病例。
ZI边缘应与重要的解剖标志(例如眼眶和翼腭窝)保持安全距离,并且两个ZI之间的间隔也应确保相邻组织浸渍和稳定植入物骨整合22。在某些情况下,为每位患者量身定制的基于计算机的指南可能存在缺陷,会降低其准确性2,26。可以应用实时手术导航系统来指导ZI的钻孔和放置。在手术导航系统的辅助下,切口范围可以在一定程度上限制在手术区域周围。此外,沿着轨迹钻孔可以避开相邻的关键结构,如眶腔和颞下窝,降低术中并发症的风险并简化手术。
在本病例报告中,使用了被动光学动态导航系统,需要在CBCT扫描期间使用牢固地附着在患者牙弓上的基准标记27。大量与植入物放置相关的研究,包括ZI放置的四联方法和三项ZI研究,表明通过在实时手术导航系统的帮助下减少术中和术后并发症,间接有效地减少了计划放置的偏差8,10,28,29,30,31.然而,在这些先前的研究中,操作员在手术前实施了六个以上具有多边形分布的基准标记。这意味着双侧上颌骨粗隆、中线腭缝合线和鼻前脊柱两侧被选为钛迷你螺钉锚固的区域32。此外,建议所有基准标记在每个区域都骨固定在多个钛螺钉上,以确保精确的配准精度。它还避免了在开瓣手术期间螺钉分裂或移动。
另一个重要程序是验证错误。在整个手术过程中进行精确验证的重要性怎么强调都不为过。验证可以分为四个级别。第一级是导航注册程序后的验证。第二级是在牙槽嵴和颧骨上定位入口点时的验证。第三级是使用颧骨手机钻孔过程中的验证。第四级是ZI实施后的验证,以确保ZI位置和方向准确。此外,在整个过程中,导航校准也非常重要。最后,操作者和手术助手都应注意参考系,以确保其稳定性,因为任何轻微的触摸都可能影响手术导航。
在本病例报告中,当植入物放置在远端位置或放置长植入物时,偏差通常似乎更大33,34。在整个过程中,很容易找到ZI,并且通过实时导航系统植入它们更安全。虽然在ZI放置实时手术导航系统的引导下限制了入口偏差、出口偏差和角度偏差,但在整个手术过程中应加以利用,以确保安全。
