Detta dokument beskriver en enkel teknik för att inducera alloantigen-specifika anergi i humana perifera mononukleära blodceller. Tekniken kan användas kliniskt för att generera icke-alloreactive donator celler. Abstract
Allogen hematopoetisk stamcellstransplantation (AHSCT) erbjuder den bästa chansen att bota många patienter med medfödda och förvärvade hematologiska sjukdomar. Tyvärr kan transplantation av alloreactive celler givare T som erkänner och skada frisk patient vävnader resultera i Graft-versus-Host Disease (GvHD) 1. En utmaning till framgångsrika AHSCT är att förebygga GvHD utan tillhörande försämring av de gynnsamma effekterna av celler givare T, särskilt immunförsvaret beredning och förebyggande av återfall. GvHD kan förebyggas genom icke-specifika utarmning av donator T-celler från transplantat stamceller eller genom tillförsel av farmakologiska immunsuppression. Tyvärr har dessa metoder ökar infektioner och sjukdomar återfall 2-4. En alternativ strategi är att selektivt bryter alloreactive celler donator T efter allostimulation genom att presentera mottagaren antigen celler (APC) innan transplantation. Tidiga kliniska prövningar av dessa allodepletion strategier bättre immunförsvar beredning efter HLA-inkompatibla HSCT utan överdrifter GvHD 5, 6. Men vissa allodepletion tekniker kräver speciella mottagare APC produktion 6, 7 och vissa metoder kan ha off-target effekter inklusive utarmning av givare patogenspecifika T-celler 8 och CD4 T reglerande celler 9. Ett alternativ är att inaktivera alloreactive celler givare T via induktion av alloantigen-specifika hyporesponsiv. Detta uppnås genom att stimulera donator celler med mottagaren APC samtidigt som blockaden av CD28-medierad co-stimulering signaler 10. Denna "alloanergization"-metod minskar alloreactivity med 1-2 stockar samtidigt patogen-och tumör-associerade antigen reaktioner T cells in vitro-11 . Strategin har varit framgångsrik anställd i 2 avslutad och en pågående kliniska pilotstudier där alloanergized donator T-celler var infunderas under eller efter HLA-inkompatibla HSCT resulterar i snabb immunförsvaret beredning få infektioner och mindre allvarliga akut och kronisk GvHD än historiska kontroller mottagare av unmanipulated HLA-inkompatibla transplantationer 12. Här beskriver vi vår nuvarande protokollet för generering av perifera mononukleära blodceller (PBMC) som har alloanergized mot HLA-inkompatibla orelaterade stimulator PBMC. Alloanergization uppnås genom allostimulation i närvaro av monoklonala antikroppar mot ligander B7.1 och B7.1 för att blockera CD28-medierad costimulation. Denna teknik kräver inte att produktionen av specialiserad stimulator APC och är enkel att utföra, kräver endast en enkel och relativt korta ex steg vivo inkubation. Som sådan kan den metod lätt standardiserade för klinisk användning för att generera celler donator T med reducerad alloreactivity men behåller patogen-specifik immunitet för adoptiv transfer i fastställandet av AHSCT att förbättra immunförsvaret beredning utan alltför GvHD.
