Summary

Abstract
检测和去除牙齿颜色的填充材料是每个牙医面临的主要挑战。荧光辅助识别技术(FIT)是一种非侵入性工具,有助于区分复合树脂材料与健全的牙齿物质。与传统照明相比,FIT是一种非常准确、可靠和快速的诊断方法。当复合树脂以大约 398 ± 5 nm 的波长照射时,某些荧光成分使复合树脂看起来比牙齿结构更亮。任何具有适当波长的荧光诱导光源都可用于这种方法。最佳情况下,这种技术无需额外的自然或人工照明即可使用。FIT的应用可用于诊断目的,例如牙科图表,此外还可用于复合树脂修复体的完全和微创去除,支架脱粘和创伤夹板去除。可以通过重叠的术前和术后扫描以及使用合适的软件进行后续计算来评估复合物切除后的体积变化。
Introduction
与传统照明(例如,通过牙科综合治疗台灯1,2)相比,FIT 的应用有助于将复合树脂材料与健全的牙齿物质区分开来。当材料发射的光波长高于吸收波长时,就会发生荧光。由于这种照明,材料看起来比牙齿3更亮。复合树脂材料的最大荧光发生在波长为398±5纳米3时。荧光在复合树脂材料中出现的是由于稀土氧化物添加到玻璃填料中,一些主要成分为复合树脂4,5.这些荧光物质的加入旨在使复合树脂的光学性能适应牙齿结构,以提高复合树脂的美学性能4,5。FIT适用于许多复合树脂材料,因为它们显示出这些荧光特性3。然而,荧光随着复合树脂材料的老化而降低6,7,8,9。

复合树脂因其操作简单、美观和临床性能而成为直接修复的首选材料13。然而,由于继发性龋齿、骨折或其他原因,许多复合修复体必须更新14,15。然而,在传统的光照条件下,去除残留的复合树脂材料可能很苛刻。即使使用放大辅助工具并使用触觉探头或对牙齿进行广泛的干燥,有时也很难将复合残留物与健全的牙齿结构区分开来。在去除粘合剂修复体期间,复合残留物的残留物会降低进一步修复体的质量,并且由于边缘可能变色而造成美学障碍1,16,17,18,19,20,21,22.相反,由于对复合树脂与牙齿结构的误诊而导致的过度制备可能导致不必要的物质损失1,2。
在牙科创伤学中,在许多情况下,使用创伤夹板固定受伤的牙齿是频繁且强制性的23。创伤夹板通常使用可流动的复合树脂材料固定在牙齿上。在这种情况下,复合树脂材料的去除不完全可能导致上述损伤。由于牙齿创伤主要发生在前牙,因此美学的损害和进一步重建的充分粘附至关重要。因此,本文的目的是演示FIT方法作为检测和去除复合树脂材料的高效直接方法的应用。

Protocol
本研究中使用的牙齿是当地伦理委员会(EKNZ UBE-15/111)批准的项目的一部分。参与者提供了书面知情同意书,所有数据都进行了去识别化以保护患者的机密性。
1. 使用FIT检测牙齿颜色的复合树脂材料
- 使房间变暗(自然光和人造光)。
- 佩戴具有紫外线防护功能的透明或黄色安全眼镜。
- 使用荧光诱导光源照亮牙齿物质和牙齿颜色复合树脂修复体(图1)。
注意:复合树脂材料看起来比牙齿物质更亮(图2)。任何波长为 398 ± 5 nm 的设备都可以用作荧光诱导光源。前照灯系统似乎特别适用,因为光斑照亮了整个口腔,并可以同时进行触觉检查。唾液和斑块等阻塞因素不会干扰FIT方法;因此,不需要事先清洁和反复干燥牙齿。
2. 使用FIT去除复合树脂粘合的创伤夹板
- 使用口内扫描设备和用于实验评估目的的适当软件进行术前扫描
- 启动口内扫描设备并打开软件(参见 材料表)。
- 按 添加新 患者以注册您的患者,填写空白姓 氏、名字、出生日期和 患者 ID,然后按 添加案例。
- 选择“适应症”部分下的“下颌扫描和印模”。然后,按下一步。
- 使房间变暗(自然光和人造光)并干燥操作区域以简化扫描过程。
- 启动口内扫描仪并对手术区域进行数字表面扫描(图3A)。
- 复合树脂材料的可视化
- 重复步骤 1.1-1.3。
- 使用常用方法(例如,带有金刚石钻头和抛光装置的高速对角手机)去除复合树脂材料(图 4)。

Representative Results
使用FIT方法使大多数复合树脂材料看起来比健全的齿结构更亮(图2和图5)。因此,FIT不仅适用于复合树脂材料的检测,而且还有助于一般复合树脂材料的去除,并且在正畸托槽脱粘和创伤夹板去除过程中明确地用于后牙1,2,24,25,26,27,28,29,30,31,32.
图6 显示了在常规照明(牙齿13,12,11)和借助FIT(牙齿21,22,23)下移除创伤夹板后的牙齿模型。 图6C 显示了软件中复合材料残留物和牙齿物质损失的量化(未改变的区域:绿色,物质损失:蓝色和紫色,多余的材料:黄色和红色)。 使用距离 工具测量的术前和术后扫描的差异显示,牙齿 13、12 和 11 中的复合残余和物质损失± 0.1 毫米。21、22和23号牙齿在创伤夹板移除后表面几乎没有变化(±0.01毫米)。此外,复合残余物通过FIT方法可见(图6B),而在常规光照明下则未被发现(图6A)。

Discussion
传统的照明(例如通过牙科治疗台灯)是识别复合树脂修复体的不令人满意的诊断工具。为了使用传统照明进行出色的诊断,需要放大辅助、干燥甚至费力地清洁牙齿。即使在理想情况下,传统的照明似乎也是不够的。一项研究表明,常规照明可能导致复合树脂修复体和健全牙齿物质33的误检。FIT方法似乎在许多方面都更胜一筹。FIT是一种具有高精度、重现性和可重复性的诊断工具1,2。即使是唾液或生物膜等阻塞因素也不会影响FIT1的结果。
FIT还显示出出色的运营商间和运营商内部协议1。研究表明,当具有不同经验水平的牙医使用FIT时,FIT可以提供令人满意的结果1,33。即使是牙科学生也显示出与使用 FIT1,33 的经验丰富的牙医相当的结果。然而,显示个体差异的视力受到许多因素的影响。在40岁以上的人群中,发生调节的生理松弛(老花眼)34。在研究中,40岁以下的年轻检查员在检测复合树脂修复体方面表现出比40岁以上组更高的灵敏度33。

在术前和术后重叠扫描后,使用软件对创伤夹板移除后的复合残余物和健全牙齿物质损失的评估说明了FIT方法的准确性。口内扫描仪适用于此目的,因为它们准确可靠28。可以高精度地检测微小的体积变化28。许多可用的复合树脂材料显示出荧光特性。然而,荧光亮度根据制造商和当代复合树脂材料的阴影而变化很大39.
值得注意的是,对于FIT方法3,36,37,39的应用,某些材料的荧光较少甚至不够。复合树脂材料的荧光信号随时间6,7,8,9而减小。使用FIT方法可能更难识别较旧的复合树脂修复体。这些因素是FIT方法的主要缺点,在应用FIT时必须考虑到这些因素。总之,FIT是一种可靠、快速且无创的复合树脂检测方法。
